However if you have not, continue with the process to find your true Root Cause. Like with a CAPA your corrective action is the "instant fix" to solve this time. You then create your "Preventative Action" to keep the problem from happening in the future.

But when someone states something, another might ask, Well have you considered this solution to the issue? You are right to state Gather Data and look for patterns. If we keep asking why, and why, we may determine the solution. People should not focus on the negative aspects. It is usually subjective. The objective way is to address this low morale issue, then that one. Seek solutions and study each issue independently.

My favorite RCA tool is the 5 whys with a twist. its simple, you keep asking why until you get to the root of the problem. For example: Initial Why: "Why do I feel undervalued by my colleagues?" Why do I feel undervalued by my colleagues? (1st Why) - They don't appreciate my contributions Why don't my colleagues appreciate my contributions? (2nd Why) -they don’t feel I have the experience. Why do they question my experience? (3rd Why) - They don’t know the deep knowledge I have. Why do I lack effective communication with my colleagues about my work? (4th Why) - I haven’t shared my successes with the team. Why haven't I proactively shared my successes with my team? (5th Why) - I am shy and feel intimidated

I have worked on several projects where we have used the 5 why’s as the root cause analysis. It has worked very well. You may need to do a lot of investigative work, asking a lot of questions even cross trades. The more people involved will help identify if it’s a person creating the low morale or part of the system. I have had excellent workers sent to me to be crew leaders in past projects that were not cut out to be a leader. This does not make them a bad worker. It just means they need a different position. It is your job to recognize that they’re still a good worker and respect them, but don’t overwhelm them with the position they do not qualify for or they will become the bad apple, and create the problem. But until you have determined what your root cause is, you cannot begin to find a solution.

Once you find the root cause of the low morale. You need to correct it ASAP by whatever means is needed . It may be sending somebody to a different position or changing the crew up . Some people just work better with other people than they do with the current crew that is working together. Do not do this in a negative manner in front of the group. It wont suffice to say because certain problems or issues originate from a junction and so doesn’t mean it hasn’t morphed and become something much more complicated elsewhere. The aim of finding a solution is to create the CAPA (Corrective Action Preventive Action) and action a follow up religiously. Then lessons learnt should be drawn up and support in terms of mentoring, coaching, teaching and establishment of various initiatives to combat future issues should created. We are trying to be transparent by solving the issues, we inspect our progress then adapt to solutions derived accordingly .
