  • Leadership from 211 San Diego/Community Information Exchange is proud to present at this year’s Civitas Networks for Health 2024 Annual Conference in Detroit with 800 attendees! Our team participated in various workshops and panels centered around transforming how people access care. Bill York, President & CEO, joined leaders from across the nation for the Pre-session Workshop, “Catalyzing Whole Person Care with Consent to Share”, where he and colleagues from California, Washington and New York provided insights and learnings regarding adopting a unified consent management. Alana Kalinowski, Director, Interoperability & Collective Impact, presented on “How Should We Develop Data and Technology Systems for Community Care Coordination”. This session explored how communities can design interoperable, multisector data-sharing systems for person-centered care. Attendees learned about the Community Information Exchange (CIE) model and gained insights on how technology partners can support health equity initiatives. Camey Christenson, Chief Business Development Officer, joined colleagues from Illinois Public Health Institute to present “Driving Collaboration and Cross-Sector Data Sharing with a Community Information Exchange” which focused on the feasibility and geographic scope of information sharing and system interoperability in support of community needs. This session provided an overview of the CIE model and showcased Cook County's initial efforts to plan a regionwide CIE to create an equitable, effective, and sustainable care coordination infrastructure. Thank you to @Civitas4Health 2024 for having us at your conference again this year! #Civitas2024
