Here's a current success story from our Patient Connect program: This patient was referred to our telehealth team due to a high risk of re-hospitalization. During the visit, the caregiver expressed concerns about a possible C. difficile infection. We discussed hydration and dietary recommendations. Our office staff arranged for a specimen collection for C. difficile testing during the next skilled nursing visit. The test result came back positive, and the patient was started on oral vancomycin. During a follow-up telehealth visit, the patient's symptoms had resolved, and they were doing much better. Year-to-date, our Advanced Patient Connect Telehealth Team has potentially prevented 210 hospitalizations. Ask us how your patients can access free telehealth visits alongside in-person appointments.
