Whether a call to her care coordinator or an update from her PCP, Sheila feels supported and valued. “One thing I like is that you actually have access. You can reach someone if you call them up. Medical care is so estranged now, it’s hard to get a connection,” she said. “When I needed help, I got it.” 🔹 We featured Sheila Yates, Housecall Providers primary care patient, in our latest newsletter. Providing the right care, in the right place, at the right time.
About us
Housecall Providers is a Portland-area nonprofit organization that believes everybody should have the right to medical care, delivered at the right place, at the right time. We offer team-based primary care wherever you call home and hospice services to be with our patients throughout the course of their lives. In addition, we provide community-based palliative care support for seriously ill members enrolled in Medicaid through our advanced illness care program. Together, all three of our programs are proving that in home medical care for seriously ill patients can improve health outcomes, increase patient comfort, reduce health care costs and contribute to care team satisfaction.

"The number of for-profit hospices has exploded from 30% of all providers in 2000 to 73% in 2020. While more patients are using hospice services than ever before, the quality and variety of care delivered by for-profits, as well as the types of patients they serve, differs sharply from nonprofit providers." The holiday season is a time for connection and reflection. It’s also the perfect opportunity to have meaningful conversations about end-of-life planning with your loved ones. These discussions may feel challenging, but they’re a powerful way to ensure everyone’s wishes are understood and honored when the time comes. At NPHI, we’ve created a 5-Step Guide for Advance Care Planning to help people begin the process of putting together their plan. Taking the time to plan ahead is one of the most thoughtful and lasting gifts you can give your loved ones this holiday season. Access the guide here: https://lnkd.in/eaYhuQ6M

Writing holiday cards for members of The Community for Positive Aging! Many of their members are isolated or have no immediate family, and a handwritten card can mean a lot. Thank you to our friends at #CfPA for allowing us to help with these the past two years. 💌

#HospiceCare focuses not just on the patient, but also their loved ones. At Housecall Providers, our hospice social workers are advocates for our patients and their families, helping to navigate end-of-life care planning, understand their treatment plan and available resources, manage stress, and be vocal about their needs. Our spiritual counselors provide emotional and spiritual support for the whole family. And our bereavement program helps loved ones as they journey through the grieving process. 🔹 Learn more about Housecall Providers Hospice: zurl.co/60Q3 #HousecallProviders #Nonprofit #Hospice #EndofLifeCare
Myth: Hospice care is only for the patient. Truth: Hospice care focuses not just on the patient, but also their loved ones. This encompasses various forms of support, such as spending time at the bedside to address families' questions and concerns, ensuring in-home caregivers receive the assistance they need, facilitating community support groups, and providing emotional care through our chaplains and social workers. Our members care continues beyond a patient’s passing, offering a year of bereavement services and personalized grief support to help families navigate their loss. #MythBustingMondays #HospiceCare

Last year, our Patient Assistance Fund covered Quiltopia entry fees for a patient who dreamt of having her quilts on display. In years prior, we've helped patients get holiday or birthday gifts for their young children. Most of the patient requests we fulfill are for items such as blood pressure monitors, compression socks, and safety items. But regardless of how big or small a request may be, this remains true: We couldn't have done it without you. To our staff that both started this fund and largely funds it, and our generous supporters, thank you so much for taking part in making a profound difference in the lives of our patients and their families. 💙 Donate or learn more here: zurl.co/LKB5 #GivingTuesday #SupportNonprofits #HousecallProviders

#SupportNonprofits In health care, there is so much that insurance does not cover. Ten years ago, our employees created the Housecall Providers Patient Assistance Fund to bridge that gap and better meet our patients where they are. Your gift to this staff and supporter funded account makes a profound difference in the lives of our homebound and seriously ill patients. Every dollar goes directly to fulfilling patient requests for essential items or one-time services they could not afford any other way. ❤️ Donate or learn more here: zurl.co/LKB5 #Nonprofit #HousecallProviders #GivingTuesday
