Proudly representing the nonprofit hospice and palliative care community.
About us
The National Partnership for Healthcare and Hospice Innovation (NPHI) is a membership organization comprising nonprofit, community-integrated hospice and palliative care providers. These members are dedicated to ensuring patients and their families have access to care that reflects their individual goals, values, and preferences. Representing providers from across the United States, NPHI and its members help design more innovative and effective models of care, advocate for comprehensive and community-integrated care customized to meet each person’s unique needs, and build collaboration between national thought leaders, decision-makers, and other healthcare stakeholders to improve hospice care. Together with our members, we ensure our most important priorities, policies and initiatives are learned at the bedside, not in the board room. To learn more about NPHI, or find a mission-driven, community-based advanced illness provider nearby, please visit our website at www.nphihealth.org, or call 844-GET-NPHI (438-6744).

Our valued member, Hospice Care of the Lowcountry, has embraced an exciting new chapter with a rebrand to MiraSol Health. This transformation reflects their expanded focus on providing a full spectrum of services, including palliative care and grief support, while remaining a cornerstone for exceptional hospice care. The rebrand reinforces MiraSol Health’s nonprofit mission and dedication to serving Southern South Carolina with the same compassion and excellence their community has trusted for years. We’re proud to highlight MiraSol Health’s innovative approach to meeting the evolving needs of those they serve.
BIG ANNOUNCEMENT! Hospice Care of the Lowcountry is now MiraSol Health. Its important to know that while our name has changed to reflect our growth and expanded services, our commitment to compassionate care, our nonprofit status, and our community roots remain the same. This new name reflects the organizations's growing range of services unified under one umbrella as we redefine our care delivery appoarch to support patients facing serious illness, terminal diagnosis, or grief, while also providing vital support to caregivers. Our core programs Hospice Care of the Lowcountry (hospice services) and Lowcountry Palliative Care (serious illness management) are now joined by Rays of Hope Behavioral health (counseling and therapy) and The Maggie Clark Volunteer Program (caregiver support). But wait theres more: we now serve Colleton and Hampton Counties and we operate inpatient palliative care through a local hospital system. New name, same mission.🌻 Learn more by visiting mirasolhealth.org. #MiraSolHealth #PatientCenteredCareFocused #YourCommunityYourChoice

Our member, Nathan Adelson Hospice, recently unveiled the Choosing Hospice platform to help Nevadans and families everywhere navigate the journey of hospice care. We're excited to amplify this invaluable resource across our network, ensuring more families have access to the guidance they need during their health journey. Early and ongoing grief support can make a significant difference in long-term mental health outcomes for caregivers and families. That’s why it’s so important to choose a hospice provider with a comprehensive bereavement and support program. Choosing Hospice stresses the importance of asking providers about their grief support services. Do they offer care both before and after a loved one’s passing? Do they provide a place for every grieving person to turn to for compassion and understanding? Hospices deeply connected to their communities recognize the value of supporting families through their grief. Our member, Stillwater Hospice, is making a meaningful impact with its specialized grief support group for the LGBTQ+ community. This safe and affirming space provides a much-needed outlet for those navigating loss, especially for individuals facing unique challenges, such as family disapproval or societal stigma. “We’re a grief-avoidant society. We don’t want people to be grieving too much,” said Bonnie Blackburn-Penhollow, Stillwater’s director of communications. This group breaks through that isolation, offering understanding and connection in a space where there’s no such thing as grieving "too much." Meeting on the second Monday of every month, the group offers coping strategies, resilience-building, and the opportunity to honor loved ones in a supportive environment. What a great way to support the LGBTQ+ community! Read more: https://lnkd.in/e83WNMX4
