When patients leave the hospital after a procedure, managing follow-up appointments, prescriptions and ongoing treatment can be incredibly overwhelming. That's where transitional care managers — a service often covered by insurance — come in. Providers can talk to their older adult patients about available options ahead of procedures, so patients don't have to navigate coordinating their transitional care when they're recovering.
