    #PBMReform PBMs Are Shifting Their Game. And Patients And Pharmacies Pay the Price! Take a close look at this chart (credit: Advisory Board). Over the years, PBMs (focusing on the big three here) have cunningly restructured their business model—and not in ways that benefit patients or pharmacies. In 2012, rebates were the largest source of PBM profits (46%), negotiated in secret with drug manufacturers. Fast forward to 2023, specialty pharmacy services have surged to dominate (39%) of PBM profits, while rebates have largely shifted to offshore GPO entities (22%) disguised as "fees" — shielded from scrutiny and conveniently outside the reach of employer audits of PBMs. Now, PBMs proudly claim they’re being transparent because they no longer retain rebates. Well, guess what? The GPOs do. It’s a clever sleight of hand that keeps the money flowing while dodging accountability. What does this mean? PBMs aren’t just middlemen anymore—they’re vertically integrated giants profiting from every angle of the system. From mail-order to specialty pharmacy to GPOs, their grip tightens while patients face higher costs. PBMs’ ability to adapt isn’t a bad thing in itself, but their focus remains solely on profits, not patients.     #PBMReform Imagine you’re buying a snack from a vending machine at your school, but there’s someone standing in front of it—a middleman. This person tells you: "You can’t buy the chips you want unless I say so." "Oh, and by the way, those chips will cost a lot more now." Welcome to the world of Pharmacy Benefit Managers (PBMs). Here’s how the game works, vending machine style: 1. Discounts That Aren’t Really Discounts The middleman tells the snack maker (drug manufacturer), "If you give me a discount, I’ll make sure these are the only chips people can buy." The snack maker agrees but secretly raises the price of the chips to cover the "discount." Now the snack looks even more expensive than before! 2. Extra Fees for Managing the Machine The middleman doesn’t just charge the snack maker—they charge the school and the vending machine manager too (that’s your employer and your local independent pharmacy). They claim it’s to help reduce costs for the school by managing what snacks the vending machine can carry. But in reality, it’s just so they can push the snack makers who pay them the most. 3. Pocketing Secret Rebates Finally, after you and your school (employer) overpay for your chips (medications), the snack maker (drug manufacturer) secretly hands some of that money back to the middleman (PBM) as a "thank you." But does the middleman pass those savings on to you? Nope—they keep it. The result? You pay more, your school has to scale back the after-school activities, the vending machine struggles, and the middleman profits. This isn’t just a snack problem. It’s a system that makes life-saving medications unaffordable for millions. You might ask, why do we even need these middlemen? Great question! They’ve built a system that benefits them, not patients or pharmacies. 💡 It’s time to demand transparency and build a system that puts patients first. It's time we fix this broken model that’s bleeding patients and independent pharmacies dry. And it all starts with building awareness. So, please check out what organizations like Pharmacists United for Truth & Transparency and NCPA are doing on this front.     Calling all employers and healthcare stakeholders! The latest National Alliance of Healthcare Purchaser Coalitions Pulse of the Purchaser report highlights a growing frustration with Pharmacy Benefit Managers (PBMs) and their anti-competitive practices. A staggering 99% of employers believe that drug prices are a significant threat, and 90% flagged PBM conflicts of interest as a major threat to affordability. It doesn't get more clear than that! From hidden fees to opaque rebates, PBMs are inflating drug costs and impacting patients, employers, and independent pharmacies—vital parts of our communities. Now, more than ever, we need transparency and accountability to ensure that PBMs serve employers, patients, and pharmacies fairly.     I don't typically share much on this forum but recently I've had some interesting interactions with various movers and shakers in the pharmacy world. I applaud those of you who've stepped up to the plate and found your voices to speak out and speak up for change in our industry. I challenge those of you who don't feel as comfortable doing so to change that right now, in this very moment. We have before us the most comprehensive PBM Reform legislation ever introduced and if you haven't seen it, you should be energized after this to stand up and speak out. I have found myself regularly frustrated by programs and ideas pushing for provider status, enhanced clinical programs, and medical billing when we haven't even fully addressed or fixed the number one issue at hand that plagues over 90% of the makeup of our businesses. That problem is reimbursement. Plain and simple. What legislation will do that? HR 9096 - Pharmacists Fight Back Act. If you own a pharmacy and you want to offer additional services now or in the future, don't you think you should be reimbursed fairly? I'm not sure how anyone thinks we'll be able to provide these services when our doors are closed. Legislation like HR 9096 that doesn't have to wait 2, 3, or 4 years to go into effect is the ONLY way we can continue to serve our communities. Get on this train and contact your US Representative and ask them to cosponsor HR 9096 or get out of the way because there is work to be done. If you aren't at the table, you're on the menu.
