This month's Wills Eye Resident Case Report reviews the case of a 71-year-old woman who presents with left upper eyelid swelling and ptosis that has progressively worsened over four weeks. She has no associated pain or vision changes, and her symptoms haven’t improved with oral antibiotics prescribed by her PCP. She’s referred to Wills Eye Hospital for evaluation by an ophthalmologist. Ocular and medical histories are notable for POAG, hyperlipidemia and hypertension, all of which she is medicated for. The ophthalmic exam revealed complete ptosis of the left eye and inability to spontaneously open the eyelid, as well as a mild supraduction deficit of the left eye. Pupils were normal, VA was 20/20 OD and 20/25 OS and IOPs measured 16 mmHg. Fundus exam was notable for c/d ratios of 0.9 in each eye (consistent with POAG history). What’s your diagnosis? A 64 y/o male was referred to Wills Eye Hospital Ocular Oncology clinic for a pigmentary lesion on his left eye. He had seen an ophthalmologist three months prior, at which point the pigmented area had already been noted for approximately four months. The patient denied any vision changes, pain, redness or discharge. The anterior segment exam OS revealed a diffuse (30 x 20 mm diameter) area of ill-defined gray pigmentation of the inferonasal bulbar conjunctiva, extending from the inferonasal limbus and involving the tarsal conjunctiva, plica semilunaris and caruncle. There was no skin, scleral, iris or choroidal involvement, and AS-OCT imaging and B-scan ultrasonography OS were unremarkable. What’s your diagnosis?
