At 34, Caroline faced a 𝘀𝘁𝗮𝗴𝗲 𝟯 𝗯𝗿𝗲𝗮𝘀𝘁 𝗰𝗮𝗻𝗰𝗲𝗿 𝗱𝗶𝗮𝗴𝗻𝗼𝘀𝗶𝘀. As both a patient and an Emergency Medicine Physician Assistant with 18 years of experience, she saw firsthand how patients are essentially "dropped off a cliff" after hospital discharge. The numbers are staggering: • Average time to follow-up in the US: 𝟮𝟵 𝗱𝗮𝘆𝘀 • Most critical intervention window: first 𝟰𝟴-𝟳𝟮 𝗵𝗼𝘂𝗿𝘀 • Dimer's readmission reduction: 𝟲𝟴.𝟳% • Patient satisfaction (NPS): 𝟵𝟰 Her solution?
