We've managed the Medicaid benefits for Florida Medicaid beneficiaries since 2000 and are currently a managed care plan in Broward County. Our new SMMC contract will expand CCP’s Medicaid geographic footprint from one to 19 Florida counties, and we anticipate our membership will more than double. 💙   "Community Care Plan is thrilled to have been selected by AHCA to expand our reach and serve Medicaid members in multiple regions as well as a specialty population," stated Jessica Lerner, President & CEO of Community Care Plan. We participated in A Trusted Circle's Postpartum Health for Moms of Color event in partnership with the YMCA of South Florida. This event is dedicated to providing families with essential resources to support maternal and infant health and safety. We connected with moms and families as they spun our prize wheel to learn about infant care and win prizes like baby keys, calendars, and more. 💕🍼 We’re so grateful for the opportunity to empower families and provide tools to help them navigate their parenting journey with confidence. 💙 #MaternalHealth #Community #Philanthropy
