Did you know that social determinants of health (#SDOH)—such as housing, food insecurity, and transportation—account for 80-90% of health outcomes? 💡 New Centers for Medicare & Medicaid Services (𝐂𝐌𝐒) requirements emphasize the importance of screening patients for 𝐬𝐨𝐜𝐢𝐚𝐥 𝐝𝐫𝐢𝐯𝐞𝐫𝐬 𝐨𝐟 𝐡𝐞𝐚𝐥𝐭𝐡 (𝐒𝐃𝐨𝐇)—𝐟𝐚𝐜𝐭𝐨𝐫𝐬 𝐥𝐢𝐤𝐞 𝐟𝐨𝐨𝐝, 𝐡𝐨𝐮𝐬𝐢𝐧𝐠, 𝐚𝐧𝐝 𝐭𝐫𝐚𝐧𝐬𝐩𝐨𝐫𝐭𝐚𝐭𝐢𝐨𝐧. These influences are key drivers of health inequities, which cost the U.S. $320 billion annually and 𝐜𝐨𝐮𝐥𝐝 𝐫𝐢𝐬𝐞 𝐭𝐨 $𝟏 𝐭𝐫𝐢𝐥𝐥𝐢𝐨𝐧 𝐛𝐲 𝟐𝟎𝟒𝟎 if unaddressed. Addressing SDoH isn’t just about compliance—it’s about improving lives. With an estimated 𝟏.𝟐 𝐦𝐢𝐥𝐥𝐢𝐨𝐧 𝐨𝐩𝐢𝐨𝐢𝐝 𝐨𝐯𝐞𝐫𝐝𝐨𝐬𝐞 𝐝𝐞𝐚𝐭𝐡𝐬 𝐛𝐲 𝟐𝟎𝟐𝟗, it’s critical for providers, policymakers, health plans, and community organizations to unite in strengthening support systems for those affected by opioid use disorder. 🤝 Addressing the 𝐛𝐚𝐫𝐫𝐢𝐞𝐫𝐬 𝐭𝐨 𝐨𝐩𝐢𝐨𝐢𝐝 𝐫𝐞𝐜𝐨𝐯𝐞𝐫𝐲 is essential, especially the role of 𝐬𝐨𝐜𝐢𝐚𝐥 𝐝𝐫𝐢𝐯𝐞𝐫𝐬 𝐨𝐟 𝐡𝐞𝐚𝐥𝐭𝐡 (#𝐒𝐃𝐨𝐇) in accessing treatment. When people face challenging social and healthcare needs, they often turn to the trusted 𝐜𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲-𝐛𝐚𝐬𝐞𝐝 𝐨𝐫𝐠𝐚𝐧𝐢𝐳𝐚𝐭𝐢𝐨𝐧𝐬 (#CBOs) in their area. 𝐒𝐨𝐜𝐢𝐚𝐥 𝐜𝐚𝐫𝐞 is a cornerstone of community well-being, yet many individuals still face barriers in accessing the help they need. 𝐏𝐫𝐨𝐯𝐢𝐝𝐞𝐫𝐬, 𝐡𝐞𝐚𝐥𝐭𝐡 𝐩𝐥𝐚𝐧𝐬, 𝐠𝐨𝐯𝐞𝐫𝐧𝐦𝐞𝐧𝐭 𝐚𝐠𝐞𝐧𝐜𝐢𝐞𝐬, 𝐚𝐧𝐝 𝐧𝐨𝐧𝐩𝐫𝐨𝐟𝐢𝐭 𝐨𝐫𝐠𝐚𝐧𝐢𝐳𝐚𝐭𝐢𝐨𝐧𝐬 play a critical role in addressing these challenges, and innovative tools like 𝐀𝐬𝐬𝐢𝐬𝐭𝐚𝐧𝐜𝐞 𝐑𝐞𝐪𝐮𝐞𝐬𝐭 𝐅𝐨𝐫𝐦𝐬 (𝐀𝐑𝐅𝐬) can make all the difference. An Assistance Request Form (ARF) lets people quickly request help with 𝐟𝐨𝐨𝐝 𝐬𝐮𝐩𝐩𝐨𝐫𝐭, 𝐡𝐨𝐮𝐬𝐢𝐧𝐠, 𝐨𝐫 𝐞𝐦𝐩𝐥𝐨𝐲𝐦𝐞𝐧𝐭 𝐚𝐬𝐬𝐢𝐬𝐭𝐚𝐧𝐜𝐞. They can securely share contact info, the type of help needed, a brief description, and consent to share this with a network of providers. 🔑 𝐇𝐞𝐫𝐞’𝐬 𝐰𝐡𝐲 𝐢𝐭 𝐌𝐚𝐭𝐭𝐞𝐫𝐬: 𝐈𝐧𝐝𝐢𝐯𝐢𝐝𝐮𝐚𝐥𝐬 𝐰𝐢𝐭𝐡 𝐮𝐧𝐦𝐚𝐧𝐚𝐠𝐞𝐝 𝐬𝐨𝐜𝐢𝐚𝐥 𝐚𝐧𝐝 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 𝐧𝐞𝐞𝐝𝐬 𝐚𝐜𝐜𝐨𝐮𝐧𝐭 𝐟𝐨𝐫: ➡️ 59% of total care costs ➡️ 79% of inpatient hospital admissions ➡️ 100% of emergency department visits (Source: 2022 Impact Report) By implementing ARFs, stakeholders across the care continuum can ensure that individuals can securely and efficiently connect to vital resources. 🛠 𝐇𝐨𝐰 𝐀𝐑𝐅𝐬 𝐖𝐨𝐫𝐤: 1️⃣ An individual seeking help submits the #ARF, a public-facing, multilingual form (available in 35 languages) that collects basic contact information and needs. 2️⃣ The form routes requests to 𝐭𝐫𝐮𝐬𝐭𝐞𝐝 𝐜𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲-𝐛𝐚𝐬𝐞𝐝 𝐨𝐫𝐠𝐚𝐧𝐢𝐳𝐚𝐭𝐢𝐨𝐧𝐬 𝐨𝐫 𝐔𝐧𝐢𝐭𝐞 𝐔𝐬 𝐂𝐚𝐫𝐞 𝐂𝐨𝐨𝐫𝐝𝐢𝐧𝐚𝐭𝐢𝐨𝐧 𝐟𝐨𝐫 𝐭𝐚𝐢𝐥𝐨𝐫𝐞𝐝 𝐬𝐮𝐩𝐩𝐨𝐫𝐭. 3️⃣ Individuals receive seamless, secure connections to care. Ultimately, both scenarios ensure the person needing help is seamlessly and securely connected to the right care. 💡 𝐑𝐞𝐚𝐝 𝐦𝐨𝐫𝐞 about how ARFs can transform access to social care in your community ➡️ https://bit.ly/3Y2NpR3 #SocialCare #GovernmentInnovation #UniteUs
