In a study of all Danish residents giving birth at ≥20 weeks from 1995 to 2018 except those with prepregnancy cardiovascular disease or chronic hypertension, the cumulative incidence of initiating antihypertensive medication within two years of delivery was 32 to 44 percent in those with a hypertensive disorder of pregnancy (HDP; preeclampsia, gestational hypertension) and 1.8 percent in those without HDP. The role of tranexamic acid (TXA) to limit bleeding during hepatectomy is unclear. In a randomized trial in >1,200 patients undergoing hepatectomy, an IV bolus of TXA followed by an eight-hour infusion did not reduce blood loss or transfusion needs compared with placebo. TXA increased postoperative complications (44 versus 38 percent). Venous thromboembolism was similar in the two groups, but the study may have been too small to detect differences. Yesterday’s #ClinicalChallenge question was: Which is true regarding the Society of Academic Emergency Medicine guidelines for the treatment of vomiting and severe abdominal pain due to cannabinoid hyperemesis syndrome? A. Intravenous fluids and standard antiemetics usually manage symptoms. B. In addition to usual care, give dopamine antagonists (e.g., droperidol or haloperidol) for symptom management; patients at risk for QT prolongation should have an electrocardiogram obtained prior to administration. C. Apply capsaicin cream to the abdomen for all patients. D. Give opioid analgesia to patients with severe abdominal pain. 🎯 The correct answer is: B. The Guidelines for Reasonable and Appropriate Care in the Emergency Department (GRACE-4) from the Society for Academic Emergency Medicine suggest using dopamine antagonists such as droperidol or haloperidol in addition to usual care to manage symptoms of cannabinoid hyperemesis syndrome in adult patients. Capsaicin cream may be used as an adjunct but is often less effective. Opioid medications should be avoided.
