Our partners' goals are our goals. So when the team at Joseph P. Addabbo Family Health Center needed help reaching out to patients to encourage them to schedule a mammogram, Yuvo Health Clinical Quality Outreach Manager Teresa Valerio picked up the phone to make the calls. The high-touch approach is nothing new for Teresa. She has extensive experience in patient engagement, most recently serving as a Population Health Program Manager overseeing a team of six patient care coordinators skilled in addressing diverse cultural and demographic needs. ⭐ This outreach is part of the end-to-end support we bring to our FQHC partners. It's a comprehensive approach that is delivering results, including an overall 65% improvement in quality performance across all measures for which we have full-year data for 2022 and 2023. “As a member of the Yuvo Health IPA, we now have the ability to partner and network with other, like-minded FQHCs," says Anthony Fortenberry, DNP, RN, NEA-BC, Chief Population Health Officer at Callen-Lorde Community Health Center. "We have already begun independently sharing best practices with our fellow IPA partners.” A major advantage of the Yuvo Health IPA is that FQHC members gain insights and ideas from each other - leaders facing similar challenges and opportunities. Last week, we held our IPA meeting in New York where members could leverage this learning collaborative in-person, while we also discussed performance over the past year, shared successes and strategized for what's coming up in the year. Earlier this month, Advocates for Community Health (ACH) released "The critical role of 340B at FQHCs." The report is a must-read, especially for those of us who understand the value health centers bring to their communities and the challenges they face doing so. The 340B Drug Pricing Program allows covered entities like FQHCs to get outpatient drugs at reduced prices, lowering drug costs for vulnerable patient populations. FQHCs rely on 340B not just as a cost-saving measure, but as a necessary revenue stream. As ACH says in the report, "The financial impact of these savings cannot be overstated, as they allow FQHCs to stretch their already scarce federal resources as far as possible, enhancing their services and expanding their reach in often overlooked communities." Earlier in the year, many members of our team came together in Texas for our first in-person retreat.
