#journalclub #caregivers This morning, we discussed a 🇹🇼 Taiwanese study about „Predictors of caregiver burden in aged caregivers of demented older patients“. This paper raised an important issue of aged society that both patients and caregivers were more than 65 years old. The outcome was Zarit Burden Interview (ZBI) score. In the multivariate models, more than 41 variables were included in the analysis. However, as there were only 328 patients in the study, this could cause unstable estimates. In our Roundtable discussion, the clinicians pointed out that they made similar observations in their practice: 1. the burden of caregivers increased, as the patient became more apathetic and irritable. This made caregivers no longer have the patience to handle the demented partner. 2. it could be helpful to educate the caregivers to recognise the disease „dementia“. It means their relationship with their current partner will no longer be the same. Some couples have been together for more than 60years. It’s tough to accept that the person you know so well is now a new person. In addition, it’s time for the caregivers to learn to say goodbye slowly. I somehow can share the feelings of those caregivers, as my grandma lived to age of 102 years, and passed away in 2022 😢. In her last 20 years, it took our whole family (countless energy, time and money) to take care of her. We hired a full time maid, and my aunt was full time at home to help her as well. Without those two full time caregivers, she would not have lived for so long. As our society is ageing rapidly, we need to find a better solution to reduce burden on aged caregivers. 👵🏻👨🦳

As geriatricians, our role extends beyond prescriptions and diagnoses; it's about fostering an environment where our elderly can thrive, not just survive. We're tasked with ensuring that their later years are not marred by isolation or neglect but enriched with care, respect, and companionship. So how do we bridge this gap? It's through continuous education - both for healthcare providers and society at large about the ageing process and how to support it effectively. It's through policy advocacy for better elderly care services. And most importantly, it's through everyday actions that honour our elderly - listening to their stories, acknowledging their worth, and advocating for their rights. Let us not forget that ageing is a privilege denied to many. It's time we redefine what it means to grow old in our society and ensure that every individual has the opportunity to age gracefully. After all, caring for our elders is not just a professional duty; it's a societal responsibility. Understanding the Care and Support Needs of Older People: Key Insights A recent scoping review published in BMC Geriatrics sheds light on the pressing care and support needs of older adults living at home with chronic conditions in the UK. Conducted by Abdi, Spann, Borilovic, Witte, and Hawley, this review utilized the WHO International Classification of Functioning, Disability and Health (ICF) framework to categorize findings and highlight critical areas for improvement. Key Findings: Challenges Identified: Older adults face significant physical, social, and psychological challenges due to chronic conditions. Their main care needs are in social activities and relationships, psychological health, and activities related to mobility, self-care, and domestic life. Barriers to Independence: Despite a strong desire to manage their conditions and maintain independence, older adults often encounter obstacles such as inadequate professional advice, poor service coordination, and insufficient information on available services. Gaps in Care: The review revealed a lack of targeted support for older workers and older carers, indicating areas where more focused care models and services are needed. Conclusion: Addressing these unmet needs is crucial for improving the quality of life for older adults. By 2030, 1 in 6 will be over 60 years old. However, just 25% of countries have the resources to adequately implement the integrated care that's needed to support them. Why? Ageist stereotypes still persist in health systems around the world, preventing change. The majority of older adults will also live in LMICs, which have far fewer resources at their disposal, making it difficult to adapt. What does this mean for me? The actions we take today will determine how well we're cared for later in life. Adapting care to focus on individual needs and integrating health and social support will help care systems manage this increase. Zooming out. This shift cannot happen without significant international collaboration. HICs will need to offer LMICs support to help them better prepare their health and care systems. #AgeingPopulations #Healthcare #HealthyAgeing
