Are you ready for round two of The Challengers? 👀 Our theme this time is regional scaling. 📈 🏥 Many hospitals are exploring or already in the pilot phase of setting up regional digital care pathways. This means involving healthcare providers outside the hospital, such as general practitioners, home care, and other partners, who may not have previously worked with patient telemonitoring. It also means learning to 'let go' of patients to primary care by hospital healthcare providers. To involve the various stakeholders in the regional network from the start, it's essential to clearly understand the value - the "why." 🤔 We'll work on this theme in the upcoming three (online) meetings. During the kick-off meeting on September 17th, we'll discuss this in more detail and Saskia Carli, Digital Care Advisor at Wilhelmina Ziekenhuis Assen will share her experiences with Transmural Home Monitoring. Hartford Foundation and Institute for Healthcare Improvement to improve the quality and effectiveness of care of older adults across practice settings by employing an evidence-based, person-centered framework known as the 4Ms: What Matters, Medication, Mentation, and Mobility. Age-friendly health care principles have been integrated into many care settings, but there is no established playbook for Hospital at Home programs. While the HaH model inherently addresses key principles of age-friendly care, there is room to incorporate even more of these ideas in practice—and a number of HaH program sare developing innovative approaches. As we gear up for our upcoming Care Coordination Summit in Seattle this May, I've been reflecting on the outstanding insights and connections forged at our Summit in Miami earlier this month. Our panel of esteemed experts, Dr. Say Salomon, Sandra Jenkins, Bradford Barber & Rhonda Sanders-Allamon each shared invaluable strategies for advancing patient care. Here are three key takeaways that resonated: 𝟏) 𝐄𝐧𝐠𝐚𝐠𝐞𝐦𝐞𝐧𝐭: Essential to maintain regular, meaningful engagements with VBC partners, with a unified view through real-time data access. 𝟐) 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 𝐏𝐚𝐧𝐞𝐥 𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭: A critical focus on high-risk patients to drive more personalized and effective patient engagement. 𝟑) 𝐓𝐡𝐫𝐨𝐮𝐠𝐡𝐩𝐮t: Addressing this challenge requires strategic partnerships.
