Sometimes asking the right question enables discovery of the root cause. I will be the first to say it is not easy question to answer. Why? Because there is a sense answering the first why is sufficient. But usually it is the most topical answer to the question. But with more thought and reflection like peeling an onion, taking off each layer teams, organizations get closer to the root cause. Remember topical answers leads to topical non sustainable solutions. But having the root cause enables sustainable solutions to be implemented.
