Meet our Medical Director, Dr. Peggy Marcon, paediatric gastroenterologist at Sick Kids Hospital and CdLS expert. We rely on Peggy to see all Ontario kids diagnosed with CdLS ages 0-18 and to provide consultations across Ontario and Canada for paediatrician, primary care providers and gastroenterologist on the often complex issues seen in CdLS. The most challenging issues in Ontario and across Canada for individuals with CdLS is around the diagnostics and management of GI issues. Over 90% of individuals with CdLS have some sort of GI issue yet often not diagnosed, followed or treated effectively. Most common issues that are left untreated as they often do not present the same as typical individuals include, silent reflux, eosinophilic esophagitis (EoE), volvulus and bowel obstruction. Often individuals with CdLS have high pain thresholds, present with no symptoms, lack the typical way of communicating and may present with responsive behaviours. Although behaviour can be challenging, ruling out underlying medical issues is essential. For many behaviour is their only way to communicate pain or discomfort even if they are verbal. Unfortunately, too often proper diagnostics and management have been dismissed and life threatening situations have occurred. We want to change this for Canadians with CdLS, the science exists that sets the standard for basic diagnostics and standards of care for this population.

I'm excited to share a recent narrative review on Paediatric Knee Injuries tailored for non-specialists, published in the British Journal of Hospital Medicine. I had the pleasure of co-authoring this piece with Chinmay Gupte. Our goal was to create a concise, easy-to-follow guide that addresses key aspects of managing knee injuries in children—featuring helpful diagrams and tables for quick reference. In the review, we answer some important questions: 🔍 How do paediatric knees differ from adult knees? 🏥 What types of injuries should I be vigilant about? 👩⚕️ How do patients typically present? 🩺 What should a thorough knee exam involve? 🧪 Which investigations are most helpful? 🛠 What are the core principles in management? 🧠 When should you refer to a specialist, and how can you prepare your patient for an efficient consultation? 💡 What role does injury prevention play? History of catheterization? 𝗚𝘆𝗻𝗮𝗲𝗰𝗼𝗹𝗼𝗴𝗶𝗰𝗮𝗹 𝗛𝗶𝘀𝘁𝗼𝗿𝘆: Newly married? Frequency of sexual intercourse? Type of pads usage? Use of condom or diaphragms? Pregnancy? 𝗣𝗲𝗿𝘀𝗼𝗻𝗮𝗹 𝗛𝗶𝘀𝘁𝗼𝗿𝘆: Profession? Is there history of frequent public washroom visits? Hydration status? 𝗞𝗲𝘆 𝗜𝗻𝘀𝗶𝗴𝗵𝘁𝘀: • History can help differentiate possiblity either it's upper UTI or lower UTI and complicated or uncomplicated UTI. • History of recurrent UTI indicates hygiene issues, stones or decreased host defences. {In this case, one can counsel regarding perineal wash direction, suggest ultrasound KUB or evaluate further for ongoing or recent infections} • Is the patient diabetic? Are they compliant to drugs and following diabetic diet? If not, go deeper into it. • Catheterization is a clear risk factor. In this case inform your patient and reassure along with treatment. • Spermicide is also a risk factor. Counsel your patient for perineal wash and bladder emptying after every intercourse and at night. • Heavy menstrual bleeding and prolong pad use can initiate UTI. Suggest cotton sanitary pads and a cotton underwear. Address menstrual issues. • Is your patient pregnant? Well, you need to go for safe antibiotics then. • Does your patient use public washroom often? If yes, you need to inform them and counsel regarding hygiene. • How many glasses of water are they drinking in a day? Advice good water intake.
