Do you routinely remap post PVI following Farapulse PFA? Getting clean lines of ablated tissue that meet unablated healthy tissue is challenging. The protocol recommended rotating in flower form and then basket form leaves partially ablated tissue (rainbow color) either on the roof or anterior side. Over trial and error of knowing what the post map will show, we have developed our protocol that consumes almost 12 applications for the superior veins and 10 for the inferior veins, consuming a total of 44 (sometimes 48) applications to complete the veins. The posterior wall takes 12 to 16 applications to isolate and stay isolated. That leaves us with very few to 70 applications maximum to avoid potential renal failure for the mitral annulus and CTI. Some preliminary data show caution on the annuli with PFA besides spasms, so an RF catheter may have to be added. This is becoming cost-prohibitive. The manufacturer must bundle pricing into a single case that includes both PFA/RF catheters.
