In a clear demonstration of the success of patient-centred care, almost one out of every eight phone calls made under the Martha's Rule scheme has led to a potentially life-saving change of treatment, according to NHS England data. Martha’s Rule enables patients and their families to request an immediate reassessment if they believe that their, or their loved ones, deteriorating condition is not being given proper attention. Merope Mills - mother to Martha, whose death from sepsis led to the initiative - told BBC News that "if we implement Martha's Rule nationally, we can confidently say that it would greatly improve care, change the culture and save lives." We're proud to support an NHS trust in West Yorkshire implement the scheme. One of the takeaways from the recent ESC conference was the new guidelines in management of hypertension. 'The 2024 Guidelines maintain the existing definition for 'Hypertension' as a BP ≥140/90 mmHg. However, they introduce a new category of 'Elevated BP' which is defined as a BP 120-139/70-89 mmHg. There are lots of arguments for organisations to work to improve the level of satisfaction of people with the care they receive as an inpatient in hospital. Here is another one. This study finds associations between patient experience and hospital readmission for people with chronic conditions. Patients giving the most unfavourable ratings of their experiences were found to have a 1.5 to 2 times higher risk of readmission. The challenge is to find the right levers to pull to consistently improve people's experience and to support them to leave hospital and stay well in their communities. https://lnkd.in/gi7aNfwS

Advocating for the Elderly: Special Considerations in July As July brings the influx of new medical residents, known as the 'July Effect,' our healthcare system faces additional challenges. This transition can be particularly precarious for elderly patients, who require consistent and nuanced care. At Patient Advocates of Southwest FL, I am committed to ensuring that elderly patients receive the vigilant care they deserve during this critical period. Here’s how I address the unique needs of the elderly during the July Effect: Heat-Related Illnesses: July’s heat can exacerbate health issues for the elderly, making them more susceptible to heat exhaustion and heatstroke. I advocate for preventative measures and ensure that caregivers are fully aware of the necessary interventions to keep elderly patients safe and comfortable. Enhanced Oversight During Staff Transitions: The July Effect brings new faces into medical teams, which can lead to gaps in the continuity and quality of care. I intensify my presence and oversight, working closely with both new and seasoned staff to ensure that care plans are followed meticulously and that any changes in treatment are double-checked. Proactive Hydration and Nutrition Management: During hospital stays, ensuring proper hydration and nutrition is critical, especially for the elderly. I collaborate with healthcare providers to monitor and adjust nutritional plans, advocating for individual needs based on each patient's specific health requirements. Clear and Simplified Communication: Effective communication is key, particularly when dealing with complex medical information. I explain all medical terms and procedures in understandable language, facilitating informed discussions between elderly patients, their families, and their healthcare providers. The 'July Effect' requires us to be even more vigilant in our advocacy, particularly for the most vulnerable, like our elderly population.
