Research in JAMA Health Forum underscores the benefits of better integration between Medicare and Medicaid for people who are dually eligible. People whose Medicare and Medicaid plans were more highly integrated reported higher levels of satisfaction with their health plans, were more likely to report being treated respectfully by customer service, were more likely to know how to obtain information related to their health and healthcare, and reported less out-of-pocket spending. These findings add to the growing body of knowledge on improving health for dually eligible individuals and are yet another reason I am proud to be working with the Association for Community Affiliated Plans (ACAP) and Medicare-Medicaid Integration Alliance to advance policy solutions. The pandemic is widely viewed as a windfall for insurers because of the decrease in the utilization of healthcare. It is less well understood how profitability trended in Medicaid managed care. The purpose of this report is to see how managed Medicaid health plans fared financially in 2020-2022 compared to prior years. Our findings show that while margin did increase, the Medicaid margin levels were explainable, and not abnormal relative to other lines of business experience. One of the key issues that makes it hard for provide integrated care for duals is the alignment of Medicare and Medicaid benefits and financial incentives. Usually, these benefits are aligned using the FIDE and HIDE DSNPs but this month Health Affairs published on article on partially capitated program with Vesta Healthcare, a home care agency, which integrated Medicaid HCBS and Medicare home care benefits on a provider level with a lot of success for duals.
