The road safety "industry" needs objective review and a comprehensive rebuild or the injuries and deaths will continue to rise. It's complex with many contributing factors. Nationally you have states & territories with different systems. Crash data collection, assessment and availability is jealously hoarded as the AAA campaign highlighted. You can't develop reliable countermeasures and effective campaigns without reliable data. Victoria is an example of bureaucracy running amok. The self-titled, taxpayer-funded "road safety partners" are more concerned with corporate image, careers and cash flows than with Victorians on our roads. If anyone disputes that, look at the TAC/VicPol transcripts from the latest road safety inquiry in Melbourne. The Economy & Infrastructure Committee was told Victoria leads the world with rich data sources expertly managed to produce wonderful outcomes. BUT, road trauma rises. The RSP policies obviously stopped working years ago. RSPs denies that. Recommendations the departments don't like, they simply ignore. The 1st thing to change is the system where the response to recommendations is mostly written by the RSP. 2nd is crash investigators should be independent of the RSP and have resources statewide.

Good Point Erika! Aircraft manufactures (Boeing, Airbus) design and build the aircraft. They also provide all of the approved technical publications required as well as the training needed to operate and maintain the aircraft in an airworthy condition. After that it's on the owner/operators to continue that trend. The operators have a responsibility to the FAA and the public they serve to follow the law and the program guidelines to ensure aircraft take off from one location and land safely at the other end in the same condition. That means participating in the periodic maintenance cycle. Take the aircraft out of service to perform maintenance when it's due. Do not deviate from the scheduled inspection times or dates. Do the maintenance right when it's due and follow your tech data to letter and teach your people to do the same. As to the slides, maybe these panels need to be inspected at a more frequent interval. After all opening and closing the door, arming and de-arming the door and slides, and just normal operations beat the "STUFF" out of these doors. It might take a bit more time away from operations, but it might serve to make them last longer and prevent an unintentional separation from the aircraft. Just a thought.

So, maybe it wasn't an accident?! Just kidding. Having a slide fall off a 30 year old airplane is not a Boeing problem That's a maintenance issue. Please let the lawyer know that airplane evacuation slides are inspected, re-tested, re-packed, and re-installed every three years, and airlines deploy at least one slide each year on each aircraft. Key findings from the UK's Air Accidents Investigation Branch - UK (AAIB) in its Annual Safety Review focused on the 10 General Aviation accidents resulting in 11 deaths that occurred in 2023. (Full report here: https://lnkd.in/gEc-79bq.) GA flying hours continue to trend down, but the accident rate worsens; with mid-air collisions statistically much higher than in the US (0.341 per 100,000 hours flown vs 0.092 per 100,000 hours). Similar to many other regulators and accident investigation bureaus, Loss of Control In Flight (LOC-I) is the AAIB's focus for improvement, with LOC-I the leading single factor for all fatal investigations during the year. Upset Prevention and Recovery Training (UPRT) can be used to teach pilots how to prevent and recover from unusual flight attitudes and upsets - conditions that may result in LOC-I events.
