A shame to be missing esteemed colleagues at the Buck Institute for Research on Aging for this meeting 6-7 Dec. Looking forward to comparing notes, after 17 years… GROWING UP IN LONGEVITY We’re just exiting childhood after founding what would now be described as a multidiscipliary ‘longevity clinic’ in 2007 - the Centre for Health and Human Performance (now WellFounded). I remain open to cool new names and reframes but I still can’t quite sincerely claim us to be a team of ‘longevity doctors’, though we are part of excellent professional networks named so, and an essential part of our work is personalised longevity research. But I do love the reframing and flow of attention into this preventive space. Keep it flowing. QUESTIONING WHAT LONGEVITY IS One of the questions we debated at this week’s Leaders In Health summit in London was an expert perspectives round table with Dr. Andrea Gartenbach, MD, Nachson Mimran, Matt Cook MD and Stasa Stankovic chaired by Lara Lewington and 150+ other colleagues was whether ‘Longevity’ is the Health. Is Longevity fundamentally a New Medicine already, or a rebrand of Advanced Prevention, 4P Medicine, Health Optimisation and Human Performance Science. All of which are established, even if rapidly evolving domains. Things that have changed significantly after an estimated 17,000 CPEX tests (V02 max +150 other biomarkers we look at in a proper physiology lab) and comprehensive biomarker panels later from just our little lab over 17 years is that mental well-being, aesthetics (the ‘how you look = how you feel’), and the neurophysiological signalling that happens in purpose and human connection are key areas that need to work their way into educational curricula and shared best practice. A lot of the rest still baffles me as to why it is being practiced before it’s been studied. But we will can learn from self experimentation too and real world data. Let’s hope our biases don’t cloud the value of our data. LONG LIVE GOOD MEDICAL SCIENCE I’m hugely bullish about the future of prevention and have a fairly good idea now about the value such clincial practices bring alongside great primary care. And a decent view on what’s round the corner that may well be additionally delivered with new biomarkers, analytics and interventions in the making. I also share a healthy optimism that elegant medicine delivered by caring physicians will not be consumed by the hyperbole and hubris drowning many in the commercial opportunism of the moment. A very comprehensive study! The finding that IFIH1 monoallelic loss of function significantly increases susceptibility to infections in males, including SARS-CoV-2, highlights critical immune implications. The decreased ISG15, IFNA, and IFNG expression in affected individuals indicates impaired antiviral defense due to MDA5 receptor truncation. This disruption hinders the body's ability to mount an effective immune response upon viral detection. Consequently, understanding the role of IFIH1 variants can inform targeted therapies to enhance antiviral immunity and mitigate the risk of severe outcomes in SARS-CoV-2 and other viral infections by addressing specific genetic vulnerabilities in the immune system. This gives some hope for developing a new generation of genomic-based interventions for SARS-CoV-2 and other viral infections. Nitin Saksena
