#COMPLETE #REVASCULARIZATION IN A #PATIENT WITH #OCCLUDED #BYPASS #GRAFTS AND #RENAL #FAILURE A 68-year-old patient with a history of CABG-4 performed 20 years ago presented with stable angina pectoris class III despite optimal medical therapy. The baseline eGFR was 26. The LIMA graft was patent, but the distal LAD was severely diseased. The Ao-RCA and Ao-Cx saphenous grafts were occluded. The Ao-Diagonal graft stenosis was treated with stenting, followed by RCA-CTO stenting. Finally, the LMCA and CX were successfully treated. Five days later, the eGFR remained at 26. NMOSD is a Cat II indication for PLEX and when accompanied by Autonomic dysfunction it really starts getting interesting The patient was received with a BP of 80/56 (and lower) with PR varying around the 60 bpm mark. Looking at MBS codes for TAVI Implants in Australia, it's pleasing to see the overall uptake in this therapy. Patients at Intermediate and Low surgical risk, however, don't seem to be benefitting as much as expected, with their percentage of the total implants remaining stable since the introduction of their relative codes. The key to diagnosing these patients at the Primary Care level before they become "High Risk" candidates is Auscultation.
