📢 The NUKA System of Care in Alaska I recently came across Ben Allen's post about what the UK can learn from the primary healthcare system in Alaska. Canada, like the UK, faces numerous challenges, making it crucial to learn from evidence-based examples. The Nuka System of Care in Alaska, renowned for its relationship-based, customer-owned care, has achieved significant success with a 44% reduction in A&E waits, 97% patient satisfaction, and 95% staff satisfaction. Key to Nuka's success are its integrated care teams, emphasis on community engagement, and focus on holistic, patient-centred care. Canada is drawing inspiration from Nuka, investing in projects like TeamPrimaryCare.ca and OurCare.ca, but more government action is needed to implement these models and leverage the full potential of a multidisciplinary primary care team. Immediate funding and respect for all primary care professionals are imperative for transforming healthcare delivery. Did you know that the government’s Federal Health IT Strategic Plan *does not* include the needs of senior living communities and other long-term care providers? The LTPAC Health IT Collaborative, a coalition of long-term and post-acute care providers, is calling for that to change—and we support their efforts! Highlights from McKnight's Senior Living: > The federal government should integrate the needs and requirements of senior living and other long-term care providers into its health IT strategy to address the varying needs and complexities of the residents served in those settings, as well as demands for data-sharing across the entire healthcare continuum, a coalition of stakeholders is recommending. > [The coalition calls for consideration of] the long-term care sector’s health IT needs, data requirements and information sharing with clinical partners, public health entities and families. > From 2018 to 2022, the percentage of residential care communities using EHRs increased from 36% to 48%, according to the May 2 ‘Morbidity and Mortality Weekly Report,’ published by the US Centers for Disease Control and Prevention…. By comparison, a study published in December 2023, prepared for the HHS Office of the Assistant Secretary for Planning and Evaluation by RTI International, found EHR adoption in nursing homes at 84%. > [A]s the nation moves toward value-based care, the collaborative said, the federal health IT strategy should encourage greater exchange of information across the healthcare continuum. That said, the long-term care sector largely has been left out of planning and funding initiatives involving health IT…

At Evergreen Life we are incredibly proud of how our Omnes Healthcare successfully managed demand and capacity during the busy Christmas and New Year period. By supporting our colleagues through digital transformation and leveraging the Evergreen Life app, we ensured patients received timely care while maintaining a high standard of accessibility across our primary care services. The Evergreen Life app has been invaluable, not only in helping us manage demand and capacity but also in empowering users to take control of their own health needs. 📊 Key Highlights: • 6,378 calls handled with an average wait time of just 2 minutes and 13 seconds. • Despite increased demand, all urgent cases were addressed the same day, and routine patients were seen promptly. • Routine appointment wait times remained within 5-7 days, significantly lower than the national average of 15-19 days. Our Friends and Family Test (FFT) results for December also reflect this success, with one of our practices achieving 100% of patients recommending the practice to their family and friends. 💚 This was all made possible through innovations like the Caythorpe model, which safeguarded care delivery during periods of high demand and staff absences. These achievements align with recent comments by The Rt. Hon. Wes Streeting MP on the future of primary care. As he advocates for modernising healthcare services, our experience demonstrates how digital transformation and innovative workforce solutions can deliver measurable improvements for both patients and staff. At Evergreen Life we are committed to continuing this momentum—both within our own services and through partnerships with other providers- ensuring we deliver accessible, responsive, and high-quality care to the communities we serve. #HealthcareInnovation #PrimaryCare #OmnesHealthcare #DigitalTransformation #EvergreenLife
