Shaping the Future of Prevention and Management of Chronic Conditions with Technology and Innovation The Health Promotion Board is partnering technology and healthcare companies on two pilot programmes that can provide real-time feedback and personalised insights on how to improve the health of Singaporeans. The two programmes are designed to provide personalised feedback, delivered through technology and coaching platforms that HPB developed in collaboration with our partners. Up to 6,000 Singaporeans will be involved in the programmes, which will empower individuals with tailored support, aimed to nudge positive behaviour change and help them prevent or manage chronic conditions. Mr. Choon Hong Tay, Chief Executive Officer, Health Promotion Board, said: “Health Promotion Board is constantly pursuing innovations that enable Singaporeans to better manage their own health. Our latest collaborations with Abbott and Health2Sync 智抗糖 on the DigiCoach programme, and with Google, ConnectedLife Health and Fullerton Health on the HealthTrack SG programme, will allow us to trial new solutions that deliver more timely and actionable recommendations to Singaporeans to prevent or manage chronic conditions. These solutions can be potentially scaled up in future to help more Singaporeans.” Read more about HPB’s collaborations at: https://lnkd.in/gBYC2V9J Image 1 – Under the DigiCoach programme, participants receive a weekly Continuous Glucose Monitoring (CGM) report with a Glucose Score to help them understand how their diet and physical activity affect their glucose levels.
