Is your PCN ready for what lies ahead? With changes to the additional roles reimbursement scheme (ARRS), for example by adding newly qualified GPs, its vital primary care networks (PCNs) have clear agreement if, and how, they may wish to deploy additional resources. As opportunities arise PCNs need a clear vision on how they wish to continue to work together at network level and beyond, to guide their decision making. Revisiting the direction with members, ensuring the Network agreement is up to date and that appropriate governance is in place will support PCNs to make the decisions they will need to imminently and, in the year, ahead. Times are tough. We are all feeling it… Especially allied health clinicians, most of whom have not had a pay rise (via the NDIS Price Guide) for many many years. Since 2019 I believe. Ouch! Once could be forgiven for thinking the NDIS is trying to save money. Some clinical practices are struggling to keep their doors open with the rising cost of living and increasing overheads. Some are being purchased by big nationwide conglomerates due to financial pressures. And soon we may add mandatory registration to the overheads impacting allied health providers. Myself and lots of my peers and clients are seeing new fees and charges creeping into invoices that enable therapists to claim for more than what they previously have been. And there are plenty of justifiable non face to face (F2F) claims, but also some that are just plain against the rules. Not all therapists and practices have a deep understanding of the NDIS Pricing Arrangements and Price Limits document, and so they may not actually know what they can and cannot claim for in non face to face supports. The Kingsgate team have been commissioned to undertake a major piece of work in the Midlands. The local integrated care system is under close scrutiny by NHS England as one of the most financially challenged in the country. Our involvement is because of our experience and expertise in this field. We’re fully hands-on, analysing different budgets and ways of working, to identify opportunities to reduce costs and ease the pressure. Excellent article this, showcasing direct feedback on PCNs and ARRS within a wider primary care context. In summary, it’s a mixed bag of success. Seems to depend on the locality buy-in to the PCN model, the CD leadership, and the specific ARRS roles. But then should we be surprised by inconsistency, when money and ‘loose instructions’ (with seemingly little foresight for cause and effect) are given to a stressed out, overworked sector of SMEs? Sounds like a few fundamental tweaks (like addition of GPs to ARRS) would make a huge difference.
