I am overwhelmed by the list of examples within this resource guide. Not knowing *how* to proceed is definitely not a barrier to addressing the SDOH! This framework was new to me, and I love seeing these ideas championed within OH--like, addressing upstream causes of ill health, harnessing collective action, using strengths-based approaches, and moving from a focus on illness to one on wellness. 👍👍 on this paradigm shift.
Ontario Health | Santé Ontario has a new resource guide to support its social determinants of health framework! I was happy to support the development of the framework last year, and I'm thrilled to see the amazing concrete examples under each component this year. This includes #SocialPrescribing under "listen to what matters," showing that's more than a connection to community, it's a paradigm shift toward healthier relationships. Health is more than just medical care. Last week, we published and began the first stages of the Convergence Collaborative on Social Factors of Health Blueprint for Action release! The Collaborative met for several months to come to a consensus on solutions for creating the best policy environment for local efforts and partnerships to improve household and community health by addressing social factors of health (also known as social determinants of health or SDOH). These convenings were supported with generous financial support by CommonSpirit Health, the Episcopal Health Foundation, and Kaiser Permanente. Their commitment to the Convergence process of building consensus among people and organizations with different views and priorities made the project possible. #SDOH #SocialFactorsOfHealth #SocialDriversofHealth #SocialDeterminantsofHealth
