Advance Care Planning (ACP) is about thinking ahead and writing down what's important so your loved ones and health-care team can honour your wishes if you're unable to speak for yourself. It can begin at any stage of life and can be revisited throughout your life journey. It creates peace of mind for you, and your loved ones. ACP isn't just for older adults or people with serious illnesses. It's something that everyone should consider, regardless of age or health status. How often do you ask patients and families after they received care what they wish they had known? Especially when it comes to serious conditions, at some point we all have to dig deep to become armchair experts for ourselves or our loved ones. Does your copy answer... ✍️ What it's really like to get a procedure or refuse treatment? ✍️ What to expect during recovery ✍️ What are typical and rare side effects of a therapy? Your copy doesn't just exist so people can find and choose your health system. Done well, it can be a part of patients' care journey.
