Jeffrey Hogan’s Post

I’m a big fan of Peter Atwater. He’s the author of one of my favorite books this year “The Confidence Map”. His post today on vulnerability and health insurance resonates and I highly recommend you read it especially in the context of this week’s events. The reality is that many in this country have lost agency over their lives and intensely distrust institutions, people, organizations and even whole industries that they feel disregard their needs and cause them harm. Peter’s categorization of this vulnerability is prescient and thought provoking. Why do people speak and act in unreasonable ways? Read the piece. “This brings me to health insurance.  If there is an industry where shrinkflation and shitification have collided to keep list prices down, it is here.  From higher deductibles to reductions in coverage, the industry has responded to soaring medical costs by reducing the quality and quantity of what it covers.  Moreover, it has done so in ways that are all but incomprehensible, if not unknowable, to the consumer until, often, it is a matter of life or death. Put simply, when Americans buy health insurance today, they don’t know what will be in the cereal box when its time for breakfast. Will it be full? Will it be empty? And that is breakfast today. Tomorrow, when they sit down again, they will have the same uncertainty. And therein lies the rub. When it comes to health insurance coverage today, Americans feel intensely powerless and uncertain. Not only is the list price viewed to be prohibitive, but the benefits are unknown and won’t be known until it is potentially too late.” “Just imagine how all these feelings of uncertainty and powerlessness get compounded when we, or a loved one, get sick.” #healthinsurance #vulnerability Christine Arnold Steve Schutzer, MD Chris Deacon Peter Hayes Andrew S. This week we witnessed the horrific and unjustifiable murder of an insurance company CEO. In its aftermath, we also witnessed an outpouring of highly emotional and, in some cases uncivil, responses to it on social media. In light of both, I explored the issue of vulnerability in the context of health insurance today.

Your point about many people losing agency over their lives is very astute. Hopelessness can give way to many terrible things for individuals and society at large. One contributing factor is that it is all too easy to blame outwardly than look inward. Even still, regardless of the underlying cause of suffering, it exists in too many corners of our world. Atwater’s point about uncertainty is equally true in health insurance as it is in financial markets. I do think our current healthcare purchasing and delivery system is antithetical to the concept of insurance. I fear that many of those so-called solutions will inevitably be worse than our current model. Final comment: we cannot change anything until we get ourselves right. Let us all make a personal commitment to recognizing the dignity of everyone we encounter. Small acts of decency spread widely, committed daily, have a multiplier effect that is immeasurable.

Thank you for your kind comments about my book and for highlighting my latest column this morning. Intense feelings of uncertainty and powerlessness are driving so many decisions today. The more we understand this, the more we can begin to address the cause.

Thank you for sharing Jeff. Great piece. And added bonus - I learned a new work that I will be introducing into my regular vocabulary: shitification.

Thanks Jeff. Once and awhile, I can only say "wow" and have to leave it at that. Mr Atwater tapped right into the heart of our feelings and it hurts. I will read his book now for sure.
