🔍 Understanding Procalcitonin Interpretation in Clinical Practice 🔬 Procalcitonin (PCT) is a valuable biomarker that helps in diagnosing bacterial infections and guiding antimicrobial therapy decisions. Here’s a quick guide to interpreting PCT levels: 🧪 Normal PCT Levels (<0.1 ng/mL): Indicates a low likelihood of bacterial infection. Antibiotic therapy may not be necessary. 🧪 Low to Moderate Levels (0.1–0.5 ng/mL): Suggests a potential bacterial infection but might also be elevated due to other factors like trauma or surgery. Monitor closely and consider the clinical context. 🧪 High Levels (0.5–2.0 ng/mL): Indicates a probable bacterial infection. Antimicrobial therapy may be warranted based on clinical assessment. 🧪 Very High Levels (>2.0 ng/mL): Strongly suggests severe bacterial infection or sepsis. "#Duodenoscopes Still Linked to #Superbug #Outbreaks, Disposable #Endcap Detachments" Yesterday, I discussed here my identifying more than 10,000 #adverseevent reports submitted to #FDA, just since January (2024), describing inadequate cleaning of a #colonoscope or #gastroscope. Indeed, inadequate reprocessing of a GI #endoscope poses an increased risk of infections. At least 1 patient died. -- Report #2 in #Netherlands: https://lnkd.in/dF5ftN8s A novel duodenoscope is linked to at least 2 infections of drug-resistant (#ESBL) #bacteria. -- Report #3 in #France: https://lnkd.in/dg59uSPz Two duodenoscopes are linked to as many as 10 infections of drug-resistant (#NDM) bacteria, with patient deaths. Also, more cases describing disposable #endcaps of novel duodenoscopes falling off inside patients were reported to FDA in October, including this U.S. report: https://lnkd.in/dr4uZuVh Nevertheless, FDA has not informed the public of this safety risk and how to prevent it. In 2023, I recommended that federal officials consider issuing a safety alert to provide updated guidance, raise public awareness, and reduce the risk of the duodenoscope’s disposable endcap becoming detached inside the patient and causing injury, including respiratory distress and mucosal tissue tears. 🔬✨ The Role of PET-CT in Diagnosing Infections in Cardiac Devices and Prosthetic Valves ✨🔬 We’re excited to share a new preprint by Tardu Özkartal and a dedicated team from the Istituto Cardiocentro Ticino - EOC Ente Ospedaliero Cantonale and the USI Università della Svizzera italiana Published in Heart Rhythm, this study investigates the potential of 18F-FDG PET/CT to enhance diagnostic accuracy in patients with bloodstream infections (BSI) and cardiac implantable electronic devices (CIEDs) or prosthetic heart valves. 🫀📊 🔹 Study Insights and Methodology: Cohort: This study included 27 patients with BSI who underwent PET-CT, divided into two groups: patients with CIEDs only and those with CIEDs + prosthetic valves. Challenges in Diagnosis: Differentiating between device infections and valve endocarditis is critical for determining appropriate treatment strategies. PET-CT Evaluation: PET-CT was performed after a median of 16 days post-BSI diagnosis to identify infection sources, complemented by other diagnostic tools like echocardiography. 🔹 Key Findings: Device vs. Valve Infections: In patients with prosthetic valves, BSIs were more often associated with valve endocarditis (77%) than with CIED infection. Conversely, in CIED-only patients, device infections were significantly more common (57%). Non-Cardiac Infections: PET-CT identified non-cardiac infections in 10 cases, mostly in the CIED-only group. Clinical Implications: PET-CT helped avoid unnecessary device extractions in many cases and guided antibiotic-only treatments successfully in 70% of valve endocarditis cases. 🔹 Future Directions: This study underscores the value of PET-CT in guiding personalized care for complex cases of bloodstream infections.
