Respiratory illness is the leading cause of emergency hospitalization in cerebral palsy (especially in GMFCS IV & V). It is associated with extended hospital stays and more days in the intensive care unit. The cost of emergency hospitalization in this population may range from 12-15 lacs (approximately in the Indian context), increasing the financial burden on families. It's important for healthcare professionals to be able to recognize adventitious breath sounds🫁 may include:🫁 1. Wheezing: High-pitched, musical sounds often heard during expiration, which can indicate narrowing of the airways due to conditions such as asthma, chronic obstructive pulmonary disease (COPD), or bronchitis😤 2. Crackles (Rales): These are discontinuous, non-musical sounds that can be fine or coarse and are often heard during inspiration. Fine crackles can indicate conditions like pulmonary fibrosis or heart failure, while coarse crackles may be associated with conditions like pneumonia or bronchiectasis. 3. Rhonchi: Low-pitched, continuous sounds that resemble snoring or rattling and are often heard during expiration. They can be indicative of excessive mucus in the airways due to conditions such as COPD or bronchitis.😮💨 4. Stridor: A high-pitched, crowing sound that is typically heard during inspiration and can be a sign of upper airway obstruction, such as from a foreign body or swollen tissues in the throat.🌪 5. Despite my best efforts, the patient's condition continued to deteriorate.... I want to take you through a recent experience in the emergency department. In the ER, every case is a story of life and death. A delicate balance where quick decisions, team collaboration, and extensive medical knowledge can save a life. My team confronted a similar situation. Last Thursday a 60-year-old male was rushed into our emergency department. His stats were alarming: - fever - rapid heart rate - fast breathing - visibly drowsy Our team jumped into action, adhering to the 'sepsis protocol'. Despite our efforts, the patient's condition did not improve. His oxygen levels dropped, and his breathing became more labored. It was clear that his life was hanging by a thread. It was clear, that a decision had to be made. I decided to insert a tube into the trachea to aid in breathing. This is never an easy choice and action, but it was necessary to ensure his survival. Once the patient was stabilized and intubated, we prepared for his transfer to the Intensive Care Unit (ICU). This transition is more than just moving a patient from one room to another; it's a complex process of ensuring continuous care and seamless communication between teams. In the aftermath, as the adrenaline subsided, our team reflected on the case. This experience was a stark reminder of the reality of emergency medicine – every day brings new challenges, and every patient impacts our collective experience and knowledge. This is what I learned that day: 1: Teamwork is critical – ER and ICU 2: Having the collective experience to provide the best care 3: The ability to learn from this, so we can recognize it early and treat it accordingly. After going through it all, I couldn't stress enough, the whole team felt burned out. If experienced professionals like me with 20+ years of experience could feel like this, then I am not sure, how the practicing doctors, who are fresh out of college would have felt. ICU Delirium: Part 6 If there was a drug that could prevent ICU delirium, we would have 100% compliance. The only tools we have to prevent delirium are: - REAL sleep - Family Engagement - Mobility - Avoiding medications that cause delirium Ironically, in most ICUs, patients that are at high risk of delirium are automatically started on deliriogenic sedatives after intubation. This makes sleep, family engagement, and mobility impossible. Wes Ely had a point when he called our ICUs "Delirium Factories" during COVID.
