Answer to yesterday’s Poll: 9-12 months to “return to sport” is the AVERAGE after Bridge Enhanced ACL Restoration/Repair/BEAR surgery (as well as ACL Reconstruction/ACLR surgery) HOWEVER, my experience is usually the athlete does not hear 9 to 12 months but hears “9 MONTHS”, and the reality is most do not get back until the 11 or 12 month point and even that is not a guarantee. Certainly some people can get back sooner, but those are usually athletes who have had an amazing strength base already, had really good neuromuscular control prior to their injury, they’re well balanced, their mechanics are good, all prior to the injury. Many athletes, who tear their ACL did not have good mechanics, strength, had strength imbalances, did not have good form, etc., so they have to learn all of that after they have their surgery, which is much more difficult, and takes longer, but they don’t understand that. When it comes to special testing at the ankle the odds are good that you will find the right diagnosis... As the tests are quite good! They are fairly accurate and therefore help you to differentially diagnose. But only if you pick the right tests... And do them in the right order!!! You should have a deliberate plan when it comes to special testing the ankle. There are a few reasons for this... None more so than the fact that some special tests are more provocative than others. Typically, you want to do the least provocative tests first... So you don't stir up unnecessary symptoms, resulting in loads of false positives... And uncertainty as to what is actually going on. When planning special testing, I use something called my special testing 'ladder.' Essentially it is a tool to structure your special testing sequence so you not only do the right special tests... But also structure your assessment so you do the right tests in the right order. Even the best special tests are not that good if they are not done properly. Want to learn more? Maybe it is because of injury, recent or chronic… Or maybe your patient is hyper-mobile? Whatever is causing joint ‘laxity’ there can be problems associated with this… Like poor rehab outcomes… And an increased likelihood of future injury. So what can you do? Here’s some tips to help you out… Using a case of a ‘lax’ ankle to give you some real-life context. >>> Artificial Passive Joint Stability If your patient lacks passive joint stability (stability from the ligaments, capsule and/or the joint itself) can you create some? This might include the use of tape or a brace for certain activities. I did this for the lax ankle athlete I have documented recently and strapped her ankle for both training and games… And purchased her a reusable ankle support for activities away from the club like walking the dog (she often rolled her ankle walking the dog!). >>> Go After Joint Stability+++ If any patient or athlete has compromised passive joint stability you need to ensure the active structures around that joint are working well. Ensuring adequate strength, proprioception and neuromuscular control is key. At the ankle this may include specific foot intrinsic activation and strengthening, ankle strengthening (gastroc/soleus) and hop and land retraining. >>> Consider What They DON’T Need A lax ankle (or indeed any lax joint) does not need more mobility… So refrain from giving your patient’s these type of exercises! >>> Consider Plan B If conservative rehab of your ‘lax’ patient does not work… What is the next step? Imaging, injection therapy, consultant review? Bear this in mind and in the case of NHS patients, if you suspect that they may need this sort of thing down the track… Given the current lengthy waiting lists it might be better to refer them earlier, to get them in the system… So if Plan A does not work and conservative rehab fails… You already have things in place. “I rely on the online videos for my rehabilitation” I wish to NEVER hear this from anybody! Either you are an athlete recovering from sports injury or someone healing after surgery, an effective rehabilitation is the source to transform your pains into potential. If you think random web sources or videos on social media can help you with this, I suggest you better redo it... Being a Physiotherapist, I have seen how a well-structured rehab plan accelerates recovery and prevents future injuries. At the same time, I have also had encounters with the patients or sportspeople who neglected or altered the rehab routine and deteriorated the condition even more. There is a prominent difference between a tailored recovery plan and the common workout routines online. Why? Because, a proper rehab plan ensures the involvement of the required muscles or tendons subjected to heal and strength gain; Whereas, the workouts accessible online are just good for warming up. AND, at times can even result in injuries if not done properly... I feel like ACL re-tear rates can be reduced, if we could address this critical oversight that ACL athletes and their team (physical therapist/athletic trainer/strength coach) often make when returning to sport, is neglecting to focus on: ☑️ Developing lateral control ☑️ Lateral movement ☑️ Change of direction ☑️ Acceleration/Deceleration ☑️ Fly sprints ☑️ Lateral agility ☑️ Posterior strength/control. These elements are crucial for overall performance and injury prevention, yet they are frequently overlooked. If you have ever assessed a patient before and used special testing (so I'm guessing that is you)... You know as well as me, that those so called 'special' tests can be less than special. Some are worse than others. The shoulder special tests are notoriously bad when you consider how accurate they actually are in terms of... Sensitivity (test's ability to diagnose a positive test) and... Specificity (the percentage of people who test negative for a specific diagnosis among a group of people who do not have the diagnosis). Whilst no test is ever going to be 100% specific or sensitive to find the exact diagnosis... Clearly some special tests better than other. The key thing with special testing is that you understand that special testing is just part of your objective assessment. With many conditions you might not even need to do any special testing to find the right patient diagnosis, if you nail the other parts of your objective assessment. This is almost the reverse to what you were taught at University right? Learning all the names of those special tests so you knew what test to do for your exams for each of the main injuries. But this is not how it works in real life. Not only are the tests not that great for many different injuries... But patients rarely present with a really clear injury. They often have other injuries going on that can make finding an accurate diagnosis tough... Especially if you are relying too heavily on your special testing to do this! I mentioned the shoulder earlier as one of the key areas so many new grads get stuck with... Mainly because they use special tests like the empty can test and Hawkins-Kennedy test, even though they ¢#@%. So bad in fact that simple range of movement testing and identifying a painful arc with your patient is actually more sensitive to diagnose a sub-acromimal pain problem than both the Empty can and Hawkins-Kennedy test!
