A FINAL LOOK AT MEDICAID UNWINDING "When pandemic-era continuous eligibility to enroll in Medicaid ended in April 2023, no one could have been entirely clear as to how many individuals would lose their coverage, report Barrett and Greene Inc. in a new Management Update. But "nearly final numbers are now in. In total, coverage was renewed for 56 million individuals by the end of the unwinding period and 25 million people were disenrolled, according to a September 18 issue brief from the George Kaiser Family Foundation. Through this analysis, one message comes through loud and clear: It matters where you live.  According to the study, five states had disenrollment rates that exceeded 50%: Idaho, Montana, Oklahoma, Texas and Utah. At the other end of the spectrum, five states had disenrollment rates under 20% -- California, Connecticut, Maine, Oregon and North Carolina. As we prepare for a new congress and presidential administration, we can expect big changes. What exactly those changes will be is harder to pinpoint. When it comes to Medicaid, here's how I'm thinking it through:

Georgians, like everyone, need accessible, high-quality healthcare to live and thrive. Yet, Georgia remains one of the few states left to expand Medicaid - instead offering an alternative program that is serving fewer than five thousand of the people in need. So many states have expanded Medicaid with success, connecting more people with the coverage they need. But Georgia’s alternative program leaves hundreds of thousands of Georgians in the coverage gap and more than 90% of its spending has gone to administrative and consulting costs. This article from Georgetown University Center for Children and Families does a great job breaking down why Georgia’s plan isn’t working, and what would work instead. In her new Forefront article, Laura Keohane from Vanderbilt University discusses how, as policy makers consider how to expand integrated Medicare and Medicaid coverage among dual-eligible beneficiaries, it is worth considering what lessons might be learned from strategies for expanding Medicaid home- and community-based services (HCBS). "Most HCBS benefits such as personal care services, adult day center services, and assisted living benefits are offered at state’s choice. Even though the Olmstead decision may motivate states to expand HCBS to avoid legal liability, there is no federal legislative mandate for states to offer comprehensive HCBS. The steps that the federal government took to promote HCBS expansion—accommodating gradual change, optional use of managed care, and funding formulas tied to clear, reasonable goals—may be adapted to increasing access to better Medicare and Medicaid coordination. Integration efforts could also learn from the persistent challenges that the HCBS sector has faced in finding enough workers to meet the needs of Medicaid participants."
