Worried about expensive cancer treatment? 60-70% of people have insurance coverage of under 5 lakh rupees. With the cancer cost being more than 10 lakh rupees in most of the cases, it is very important to plan the treatment very meticulously so that you can decrease your out of pocket expense. What can you do to cover your maximum journey in the insurance? 1. Get a complete idea of your treatment journey. Ask your doctors how long will the treatment go on for. What will the different treatment modalities involved, i.e Chemotherapy, targeted therapy, surgery, immunotherapy, radiation etc. 2. If you see that your coverage is less than your overall cost then ask your consulting doctors what are the ways in which you can optimise each of these cost. 3. You can reduce your chemotherapy cost by getting it done in a day care chemotherapy centre as the cost of chemotherapy centre at a day care centre is generally on a lower side. 4. You can look for hospitals which have a lower cost of the same treatment under insurance. 5. US$21 Mln Funding The pain in your back is too much to bear. Or, perhaps you fear arthritis is setting in. Whatever the cause, your physician recommended you get an MRI to have the problem examined. The only problem is that you just found out that your local hospital charges more than you can afford. A family took their 3 years old daughter for an MRI. She suffered from a rare genetic disorder and had already undergone CT scans, colonoscopies, ultrasounds and other tests. Based on those experiences, the family assumed the MRI cost would be a few thousand dollars. So, they opted for an out-of-network provider in their area and agreed to pay half of the cost themselves. I am excited to share our recent publication on patient factors associated with an early transition from hemodialysis (HD) to either kidney transplantation or peritoneal dialysis (PD) after autogenous arteriovenous (AV) access creation. After AV access creation for end-stage renal disease, most patients continue on HD; some receive kidney transplants, and a subset transition to PD. Our study aimed to identify patient factors linked to early transitions from HD to kidney transplantation or PD. Methods: This case-control study analyzed data from the Vascular Quality Initiative (2011–2022) focused on patients with first-time AV access creation and long-term follow-up. Control group patients remained on HD, while the case groups included patients who received early kidney transplants or converted to PD. Univariable analysis showed variation in transplantation and PD conversion rates with race, insurance status, area deprivation index (ADI) quintile, and medical comorbidities. Multivariable analysis linked impaired ambulation, current smoking, non-commercial insurance, Black race, heart failure, higher BMI, and older age to decreased transplantation rates. Conversion to PD was less likely with impaired ambulation, Hispanic ethnicity, Black race, former smoking, controlled diabetes, and older age, while higher ADI quintiles increased conversion rates. Black race and non-commercial insurance were associated with decreased kidney transplantation rates, highlighting disparities beyond community-level access to care. Early kidney transplantation offered a 3-year survival benefit over HD and PD, which had similar survival rates. Efforts are needed to improve access to kidney transplantation and PD.
