Here are some key points about a healthcare proxy: 1. Legal Document: A health care proxy is a legally binding document that must be signed in accordance with state laws. Requirements for witnessing or notarization may vary by jurisdiction. 2. Decision-Making Authority: Your healthcare agent has the authority to make decisions regarding your medical treatment if you are unable to communicate your wishes, such as if you're unconscious or incapacitated. 3. Scope of Authority: The scope of authority granted to the healthcare agent can be specified in the document. This can include decisions about medical treatments, surgeries, medication, and end-of-life care. 4. Advance Directives: A health care proxy is often part of an advance directive, which may also include a living will. A living will outline your preferences for end-of-life care, such as whether you want to receive life-sustaining treatments under certain circumstances. 5. Choosing a Proxy: You should choose someone you trust implicitly to act as your healthcare agent.
