Long post colleagues - yet worth the read. The underrepresentation of women in clinical trials is a significant and an ongoing issue in medical/health research, with serious implications for women's health and overall treatment efficacy. This disparity stems from several factors: Historical Context. Historically, women were excluded from clinical trials due to concerns about potential risks to reproductive health and pregnancy. These concerns led to policies and practices that favored male participants, resulting in a knowledge gap regarding how different treatments affect women. Biological Differences. Men and women can respond differently to the same treatments due to biological differences, such as hormonal variations, metabolism, and genetic factors. The lack of female participants means that many drugs and medical treatments are primarily tested on male bodies, leading to potential inaccuracies in dosing, efficacy, and safety for women. Regulatory and Policy Issues. While there have been efforts to address this imbalance, such as the U.S. National Institutes of Health (NIH) Revitalization Act of 1993, which mandated the inclusion of women and minorities in NIH-funded clinical research, compliance has been inconsistent. Historically, there has been nowhere else to go for younger adults living with conditions like traumatic brain injury, HIV/AIDs, serious injuries due to motor vehicle accidents, drug overdose, hemiplegia, quadriplegia, severe mental illness and neurodegenerative diseases. The significant increase in younger people moving into nursing homes followed the Medicaid Expansion of the Affordable Care Act, which covers nursing home services for younger adults. Although they now have a place to live for a short- or long-term stay, it remains unclear if younger adults benefit from this type of care setting and if their needs can be met. ATTENDING THE INDY 500?🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️🏎️ #PREPARE #BEAWARE (NEJM Journal Watch) Between 2016 and 2020 in Indianapolis, chlamydia was diagnosed in 52,946 patients, of whom 19% were seen in the public STI clinic and 81% by private providers. Gonorrhea was diagnosed in 25,699 patients, 24% of whom were seen in the public clinic and 76% by private providers. For chlamydia, 18% of patients did not receive treatment, including 6% of those seen by the public provider and 20% of those seen by private providers; however, choice of treatment followed the current guidelines in both public and private settings. For gonorrhea, 26% of patients were not treated, including 5% of those seen by the public provider and 33% of those seen by private providers. Almost all patients seen by the patients were not treated, including 5% of those seen by the public provider and 33% of those seen by private providers. Almost all patients seen by the public provider received guideline-driven therapy, compared with 79% of patients seen by private providers. 🔥🔥🔥COMMENT🔥🔥🔥 The poor performance of these private providers in treating chlamydia and gonorrhea is worrisome, especially as public health clinics have suffered closures and budget cuts despite increasing rates of infection. Appropriate choice of therapy is especially important for gonorrhea, which has developed resistance to multiple antibiotics. We need to bolster public health infrastructure to maintain sexual health.
