A 67-year-old male was admitted to the hospital with right lower extremity pain and hypoesthesia, especially on the left L5 dermatome. MR images showed extruded herniated nucleus pulposes (HNP) at the L 4-5 level. He was recommended for surgical treatment but he refused. As such, he was treated conservatively. The pain was relieved gradually and followed up as an outpatient. Nine months later, the pain disappeared and the hypoesthesia was weakly remained. Follow-up MR images showed no HNP. The spontaneous regression of CDH is thought to be caused by three mechanisms. The first mechanism involves dehydration and shrinkage of the herniated nucleus pulposus. For the second, there is a retraction of the protruded disc. In the third, there is enzymatic degradation and phagocytosis of the extruded/sequestrated disc material due to an inflammatory reaction and neovascularization.

Here is the reason why it is important to get a contrast enema before starting bowel management. During bowel management week, we are monitoring the amount of stool in the rectum and left colon. The right colon always has some stool because that is the beginning of the colon. In some patients, the beginning of the right colon (cecum) can overlap with the rectum in the pelvis and it may give the false impression of stool in the rectum.
