Intracranial stenting for atherosclerosis with high grade stenosis in blood vessels of the brain remains a treatment option for carefully selected patients, it is a challenging condition to treat with a high degree of morbidity if not treated timely and yet stenting the artery has its share of risks. While most patients are managed medically and often they are able to stabilize on medications alone, some selective cases need interventional management. Sharing a case treated at Memorial Hermann Health System The Woodlands Medical Center, young patient admitted with unilateral body weakness and loss of speech, high grade near occlusive stenosis in the left middle cerebral artery - supplying blood to the speech and strength center. Despite maximizing medical therapies patient remained very blood pressure dependent with return of symptoms each time blood pressure lowered. After a multidisciplinary discussion, decision made to treat stenosis with intracranial stenting using Medtronic Resolute onyx frontier balloon mounted stent (off label). Excellent patency of blood vessel post deployment of stent , intentionally landed it proximal to the bifurcation to avoid jailing the branches, patient did very well immediately post op as well as on followup in clinic with normalization of blood pressure. CCTA disrupts traditional coronary diagnostic and intervention planning approach, by providing both in-depth anatomical and physiological information in a single non-invasive examination. With CCTA emerging as the first diagnostic choice in patients with chest pain, the number of patients arriving to cathlabs with preceding CCTA exponentially increases. It is neccessary for (invasive) cardiologist to be fluent CCTA readers, which will likley translate into improved patients’ outcomes and more efficient diagnostics and treatment.

The 2023 ACC/AHA/ACCP/HRS Atrial Fibrillation Guidelines are out and there are 10 top take home messages (link below). I draw your attention to number 5, Early Rhythm Control: "With the emergence of new and consistent evidence, this Guideline emphasizes the importance of early and continued management of patients with AF that should focus on maintaining sinus rhythm and minimizing AF burden." To me this implies that diagnoses need to be made quickly and accurately. Recent evidence suggests that some ambulatory cardiac monitors are better than others at delivering diagnostic yield and reducing unnecessary delays caused by the need for retesting. Other evidence suggests that some monitors are better at defining a true AF burden than others. I will post links to to some of that evidence in the comments.

🚨 New Insights on Thoracentesis: Wall Suction vs. Gravity Drainage 🚨 Our recent multicenter, single-blinded randomized controlled trial has shed light on the long-standing debate of whether wall suction or gravity drainage is more effective—and comfortable—for patients undergoing large-volume thoracentesis. The study, involving 221 patients with effusions ≥500 mL, compared procedural chest discomfort between the two methods. Key Findings: 1. No significant difference in postprocedural chest discomfort between wall suction and gravity drainage (P = .08). 2. Secondary outcomes like dyspnea, procedure time, and complication rates were also similar across both groups. 3. The only notable difference? Gravity drainage took about 3 minutes longer. ✨ Takeaway: Both wall suction and gravity drainage are equally effective for patient comfort and clinical outcomes in large-volume thoracentesis.
