Thrilled to read the long-awaited study demonstrating the positive impact of the CF R.I.S.E. Transition program had on clinical outcomes in adolescents and young adults (AYA) with Cystic Fibrosis (CF). Congratulations to Katherine Melton and team for finally tackling the question I received from countless Center Directors and conference booth visitors for the nearly 8 years I worked on the program. While we had compelling qualitative and quantitative data from the pilot program, clinical outcomes were never studied. During my tenure I was fortunate to attend advisory board meetings, speak with care teams during program trainings and coaching calls and work closely with the Transition Advisory Council (TAC) to develop additional tools and resources. I completely agree with the authors’ findings that successful transition from pediatric to adult care, much like many other aspects of health care, is influenced by individual factors, including behavioral, emotional, and social determinants of health. What I would also underscore is the role of external support system, which was a unique feature of the CF R.I.S.E. program. The program was designed to be implemented by teams in clinic, and the tools encouraged *some* parent participation (Responsibilities Checklists), while empowering AYA to educate and advocate for themselves. I believe that the program’s success and any associated improved clinical outcomes all ultimately come down to the multi-disciplinary care teams who embraced CF R.I.S.E. and integrated it into their clinic flow. The singular common denominator among the clinicians that I worked with was their conviction that knowledge is power, and demonstrated by their commitment to improving their patients' lives by giving them the information and skills to take responsibility for their health care. https://lnkd.in/gJV8s7h3

【Advancing Pediatric Hemodialysis Care through the Novel Intervention in Children's Healthcare (NICH) Program】 Full article: https://lnkd.in/gDz_J5Fg (Authored by Jessica Williams, et al., from Lucille Packard Children’s Hospital Stanford (USA), etc.) Studies continually show that there are social determinants of health, such as structural racism, housing quality, and employment opportunities, which prevent successful outcomes for patients with end-stage kidney disease (#ESKD) on hemodialysis. Therefore, it is important to uncover how to best support families despite these barriers, restoring equity and advancing patient care. This retrospective cohort study reviews patients with ESKD on hemodialysis involved in Novel Intervention in Children's Health (#NICH) to explore if NICH serves as a liaison between the patients and multidisciplinary medical team and to explore if NICH helps patients better manage the challenges of end-stage kidney disease. #Pediatric_Hemodialysis #Healthcare_Disparities
