With a looming government shutdown set for tomorrow, no matter which side of the debate you find yourself on, there is a lot at stake. As the $100.4 billion Continuing Resolution (CR) has grown to 1547 pages, about 550 pages of the bill relates to healthcare and Title IV Veterans issues. Included in the CR is 1) A 2 year extension of telehealth waivers set to expire 12/31/24, 2) PBM reforms that ban industry compensation linked to Medicare drug prices, increased reporting requirements on fees and rebates (requiring rebates to be passed on to plan sponsors), 3) A reversal of Medicare physician pay cuts, 4) Reauthorization of the Opioid Support Act, 5) Payments to community health centers, 6) Funding for pandemic readiness, 7) Funding for the Improving Veterans Experience Act and the Regular Order for Veterans Act (requiring budgetary updates from VA) and more than 50 additional legislative changes to the Medicare and Medicaid programs. #healthcare #veterans #government #continuing resolution #governmentshutdown #telehealth

Tonight, the House of Representatives advanced a Continuing Resolution to avert a government shutdown and fund the government through March 14, 2025. The Senate is expected to pass the legislation, and President Biden is likely to sign it into law, ensuring the government remains open. Key healthcare priorities—including the reauthorization of the Pediatric Rare Disease PRV program, PBM reforms, the Accelerating Kids Access to Care Act, and the reauthorization of the SUPPORT Act—were not included in this bill. As a result, the Pediatric Rare Disease PRV program will lapse. However, it does reauthorize funding for Community Health Centers and extends Medicare's pandemic-era telehealth flexibilities through March 2025.

Important new brief out of the Bipartisan Policy Center: "The Future of Acute Hospital Care at Home" To ensure beneficiaries can continue to receive various levels of care in their preferred settings, the BPC brief provides immediate-term federal policy reform recommendations for the potential reauthorization of the Acute Hospital Care at Home model, including extending the waiver for five years, while outlining key issues for policymakers to consider as further data emerges. Recommendations largely align with: --Facilitating appropriate uptake and participation in the model; --Gathering evidence on the model to support high value, equitable, and comprehensive care; and --Ensuring program integrity and patient and caregiver safety. I love to see Medicare Advantage plans subject their results to rigorous peer review, and Humana has consistently done this. This study looks at "senior-focused primary care practices" and outcomes of their MA enrollees. Cut-to-the-chase--more primary care is associated with fewer ED visits, hospitalizations, and readmissions, and other quality outcomes are quite strong compared to a similar set of beneficiaries in TM. These outcomes are consistent with other findings, such as from the MSSP ACO program. These results don't mean that current MA payment methods are accurate, but it does mean that some MA plans have a business model that aligns with beneficiary interests and are partnering with physician groups well and paying for better outcomes.
