Trigger Training and its hazard identification is one of the most crucial concepts in maintaining sustainability in safety, quality, and reliability. At High Reliability Training (HRT), we've discovered, through analysis of multiple events, that visual cues existed 90% of the time before an undesirable event occurred. These cues inform workers that something is wrong and that there are holes in current organizational defenses. With this knowledge, we trained front-line workers on how to identify these triggers and respond effectively when they notice something is off. Our Trigger Training has received positive personal reviews: “Trigger Training is practical and informative, providing achievable measures to improve organizational performance.” “Trigger Training was so well received that a decision was made to train the entire organization. The next quarter's performance metrics for ‘conservative bias’ showed improvement.” Trigger Training is designed to help all employees recognize when they are in a state of uncertainty and it's time to STOP and seek assistance. Sometimes stopping is the best way to start. At SBM, this philosophy underpins our commitment to safety, especially during June's National Safety Month. We prioritize safety at our sites year-round through extensive training—including more than 100 SBM-produced on-demand courses—and behavioral auditing helps our associates spot hazards before they become accidents. Our Stop the Job and Good Catch processes are just two of the ways we empower our team to work safer, smarter, and more efficiently. Thanks to these initiatives, our YTD Total Recordable Incident Rate (TRIR) stands at 0.67—more than three times lower than the industry average. Join us in celebrating National Safety Month as we continue to make safety a top priority 24/7. #NationalSafetyMonth #WorkplaceSafety #SafetyFirst

Rethinking Safety: Moving Beyond the Iceberg Model Traditional accident prevention focuses on events leading to an incident, often using Heinrich's Triangle or Iceberg Model. But this approach, while once groundbreaking, is now outdated. It oversimplifies complex systems and ignores the critical roles of organizational culture and human factors. Modern safety science tells us that natural prevention requires a more profound understanding—beyond just mapping previous incidents. It's about recognizing the mindset, risk appetite, and cultural factors driving safety. In collaboration with Eskild Sørensen, we've developed a course that challenges conventional thinking and introduces a fresh, holistic approach to accident prevention. Ready to explore how to move beyond outdated models and embrace resilient safety practices? Let's connect. The training can be conducted in both English and Danish.
