🚨 Alarming Rate of Unintended Medication Discrepancies in Patients! A recent study by the NIH found that over half of patients experience unintended medication discrepancies during hospital admissions, posing serious risks. Key factors include: 1. Poor communication among patients, doctors, and nurses 2. Lack of coordination between healthcare providers 3. Illegible handwriting on prescriptions 4. Outdated medication lists To reduce these discrepancies, we can: 1. Improve communication among healthcare providers 2. Use electronic health records to track medications 3. Implement medication reconciliation programs Medication reconciliation ensures accurate and up-to-date medication lists, preventing errors and enhancing patient safety. I encourage all healthcare professionals to explore medication reconciliation as a way to enhance patient care.🏥💊 #patientsafety #medicationerrors #medication #doctors #reconciliation #healthcare #pharmd #healthcarequality

#TuesdayTip 🌱 - How often do patients need to book a follow-up appointment? We ask our patients to book in an appointment after the first month of care, so that our clinicians can make sure your treatment plan has started smoothly. After that, we ask that you book in every 3 months. Our team wanted to highlight follow-up appointments today as it is #PatientSafetyDay, which aims to raise global awareness for reducing patient harm. Having regular check-ins allows us to do this, and patients can book in calls with our Nurses and Doctors as often as they'd like. #PatientsFirst #PatientCare #MedicalCannabis #UK

Failure to administer medications as prescribed is a deviation from the standards of care. It's not just a procedural misstep—it's a serious lapse in patient care. Medications are prescribed for specific purposes. Each prescription is tailored to address particular health issues and promote patient well-being. Timing, dosage, and frequency all matter. Nurses play a pivotal role in ensuring these critical factors are adhered to for each patient. When prescribed medications are not administered correctly, potential consequences include: o Reduced effectiveness of the treatment. o Worsening of the patient's condition. o Unexpected side effects or complications. o In some cases, serious health risks. Proper medication administration is a cornerstone of quality healthcare. It's a key responsibility that directly impacts patient outcomes. Ongoing education and strict adherence to protocols are essential for maintaining high standards in medication administration. Not only is adhering to medication administration is a duty—it's a vital component of patient safety and effective care. Administering medications accurately and according to the prescribed plan is a vital part of maintaining patient safety. When this process is not followed, it constitutes a deviation from the standard of care. #PatientSafetyMatters #MedicationSafety #StandardsOfCare

It's the first Thursday of March! This means it's National Hospitalist Day, which recognizes the contributions of the more than 60,000 hospitalists nationwide. When patients are admitted into a hospital facility, hospitalists are physicians that follow the patient's care in an inpatient setting. Hospitalists ensure patients are well enough to ultimately discharge from the hospital. #CommunityHospital #Healthcare #Medicine #InternalMedicine #Hospitalist #HospitalMedicine
