Why don't things get better on patient safety and culture in the NHS? One reason is that the NHS & government do sweet FA to act on inquiry recommendations after they are made. 5moScott - I’ve heard the term word salad recently and that seems to reflect this typical NHSE document. It does very little to evidence the added value that ICBs should be bringing to improve our health care system
