Leveraging manual input and passive data from connected devices, the system provides coaching, education and medication reminders to patients. The health care team can use those insights for Remote patient Monitoring, leveraging key information such as Symptom Tracker, Weight, Blood Pressure, …, with the goal od decreasing acute decompensation events and re-hospitalizations.   💗 Heart failure is a global health issue affecting more than 64 million people worldwide.
