Today, I had the opportunity to identify two patients and perform my first Lacrifill administrations! 🌟 I am excited to offer Lacrifill as a treatment option for my ocular surface disease (OSD) patients. Lacrifill represents a significant advancement in managing OSD. This gel temporarily blocks tear drainage by occluding the canalicular system. As a refractive cataract and glaucoma specialist, effectively managing OSD is crucial for achieving optimal visual outcomes post-cataract surgery and enhancing the tolerability and compliance of glaucoma medications. Our first patient, a glaucoma patient post-filtering surgery, wanted to discuss ocular discomfort from dry eyes rather than his glaucoma progression. Traditional punctal plugs were not viable due to chronic lid changes from long-term glaucoma medication usage. Lacrifill provided a much-needed alternative to alleviate his discomfort. Our second patient, a glaucoma patient scheduled for FLACS with a toric IOL, had significant OSD. We escalated dry eye therapy with Lacrifill and plan to repeat biometry measurements prior to surgery, ensuring the best possible surgical outcomes. Managing OSD is vital for quality of life, visual outcomes, and overall patient comfort. An exciting procedure was recently performed at UH Cleveland Medical Center in Cleveland, OH. Dr. Gabriel Smith and Dr. Dana Defta treated a patient who presented with moderate to severe Degenerative Disc Disease (DDD) with foraminal stenosis at L4-5 and L5-S1, and a lateral and superior disc herniation on the left side at L4-L5. The surgical team planned and executed an L4-S1 minimally invasive surgery for transforaminal lumbar interbody fusion (MIS TLIF) with navigation to address both levels. The team leveraged the NuVasive Tube System for minimally invasive access, paired with MOD-EX PL expandable interbodies for customized lordotic and posterior height expansion. The low-profile design of the expandable technology enabled the team to add 2mm of posterior height to the disc space, directly decompressing the foramen and neural elements to help relieve the patient's symptoms.
