Creeping change: the silent disruptor in process safety. When we talk about process safety, our minds often jump to catastrophic failures or dramatic incidents. Yet, it's the incremental, barely noticeable shifts - the creeping changes - that often pave the way for potential disasters. Imagine this: a small alteration in materials, a slight adjustment in temperature, a minor tweak in operating procedures. Individually, they seem benign. But cumulatively? They can significantly alter the risk profile of an operation, often bypassing the rigorous safety checks that a more noticeable change would trigger. This phenomenon isn't just an oversight; it's a systemic issue. Our processes for managing change are built around the assumption that changes are discrete, significant events that can be captured, reviewed, and controlled. But what happens when change doesn't announce itself? When it's the result of multiple, minor decisions that, over time, erode our safety margins? The challenge is twofold. First, recognising that these small changes are occurring and understanding their potential cumulative impact. Second, adapting our management of change processes to capture and evaluate the safety implications of these creeping changes. The solution? Vigilance and adaptation. We must embed into our processes the ability to detect and assess the cumulative impact of minor changes. This requires a culture that values continuous observation, questioning, and reporting of even the smallest deviations from the norm.
