A lot to unpack here. First. Greater than 90% of reimbursements are from FFS. As long as payers reimburse using this predominant method, FFS wins. True VBC is maximizing quality input and output with decreased cost. As an example, the number of total joint replacements people will need is increasing as the population ages. Where is the value with increased demand? Continued efforts toward same day discharge regardless of whether the procedure is done in an ASC or hospital. Fewer days in the hospital decreases cost. Second, focus on the periop patient experience using education, enhanced recovery after surgery programs, and providing excellent post op care and follow up. The volume and demand is already baked in. Doing it better and with less money is the value. But it takes a village to do it right with teamwork, collaboration, and a laser focus on continual improvement.

Pssst, it’s not fee for service (ffs) that’s devastated American Healthcare access, outcomes or costs. The Bismarck Healthcare Modelused in several democratic capitalistic nations such as Germany, Japan, Switzerland and others enables private insured and private ffs manufactured medical care for all citizens at a fraction of the cost and with better outcomes and access than in America. Bismarck systems have more private hospitals and health insurers COMPETING per capita than in America.
