❓ How can home health and hospice agencies be sure their patients are in the right care setting for them and their families? 🔎 Take a look at our latest article to find out! https://lnkd.in/gqupD2Jr Coordination of patient transitions and understanding which patients are best suited to hospice care can be challenging. Too many patients who would benefit from hospice move too late to fully benefit, or don't transition to hospice at all. Instead, they spend more time in home health than is advisable given their condition, resulting in insufficient care. Patients whose trajectory shows that they are approaching end-of-life care should be identified early to facilitate conversations with family and patients about hospice resources, compassionate care, and end-of-life planning.
