Are you ready for round two of The Challengers? 👀 Our theme this time is regional scaling. 📈 🏥 Many hospitals are exploring or already in the pilot phase of setting up regional digital care pathways. This means involving healthcare providers outside the hospital, such as general practitioners, home care, and other partners, who may not have previously worked with patient telemonitoring. It also means learning to 'let go' of patients to primary care by hospital healthcare providers. To involve the various stakeholders in the regional network from the start, it's essential to clearly understand the value - the "why." 🤔 We'll work on this theme in the upcoming three (online) meetings. During the kick-off meeting on September 17th, we'll discuss this in more detail and Saskia Carli, Digital Care Advisor at Wilhelmina Ziekenhuis Assen will share her experiences with Transmural Home Monitoring.
