Adalyn, my first daughter, may have benefited from IDbyDNA's Precision Metagenomics Solutions; she passed away 6yr today after ~6mo fight in the NICU
Today, on August 23rd, 6 years ago in 2015, I held my oldest daughter in my arms as I read her Psalm 23 and she peacefully passed away after an almost 6 month fight in the Neonatal ICU at Rush University Medical Center, as I worked as an Infectious Disease Pharmacist at the time. Adalyn was born about 3 months early on March 15th, 2015, and was a 27 weeker born 12 inches long and only 1 pound 7 ounces, < 5% on the weight scale even compared to other micropreemies. Addie had >100s if not >1,000s of care givers, from the Neonatology Clinicians (Attendings and Advanced Practice Providers), Nurses, Dieticians, Social Workers / Case Managers, Respiratory Therapists, Lactation Consultants, Specialist Consultants, Chaplains, Physical Therapists, Developmental Therapists, Speech Therapists, Palliative Care Medicine Specialists, etc. We are forever grateful for these extraordinary professionals that allowed us for more time with our angel.
As a parent of a premature infant, I know first-hand how hard it is to stay ahead of challenges on the horizon, and having the most amount of information available timely to aid her inter-disciplinary team in making the best patient care decisions. I envision that my new company, IDbyDNA, offers precision metagenomics solutions to equip microbiology laboratories to inform patient care decisions across healthcare settings in the US and around the globe. If we would have had the answers with improved viral pathogen detection for her lungs, it would have given me more information and closure with projections on the next stage of her course, as often her respiratory cultures were negative. Additionally, when we woke up on July 4th, 2015, instead of celebratory fireworks, I was viewing gram-negative rod fireworks that ended up being identified as Pseudomonas and Klebsiella. She was septic, and discussions were had to add a nephrotoxic agent to double cover, gentamicin, or broaden her anti-pseudomonal beta-lactam. The time it took from pathogen ID to phenotypic susceptibility data seemed like forever to me, as I was concerned since I rounded on infected adult patients in the hospital, I could have colonized her and put her at higher risk for a more drug-resistant infection associated with worse outcomes. Having antimicrobial resistance (AMR) markers detected at 24 hours would have saved me a few extra sleepless nights to know that her Pseudomonas and Klebsiella were not multi-drug resistant pathogens.
I also wonder, if getting the identification of pathogen and AMR results sooner when I was in the ICU being treated with antibiotics with toxic shock syndrome when I was 17 years old, would have better informed the antibiotic decisions made and improved my course. At any rate, my Mom told me about how the Infectious Disease Pharmacist explained my vancomycin dosing on daily rounds that week, and it set me on my career path from that moment forward.
Just some food for thought, and a reminder to tell your loved ones how important they are to you today, and every day. My husband and I are headed to the cemetery with our 2 healthy living children, Kylie and Hunter, to visit their “big” but little peanut sister, Adalyn Rose.
